Interim 10CFr.50.55(e) Report: Limitorque Motor Operators

Dear Sir:

In Reference (b), we submitted a written report on four deficiencies regarding Limitorque motor operators. Two of the deficiencies were determined to be reportable per 10CFR50.55(e). The reportaLility of the two remaining deficiencies is still under evaluation.

On August 20, 1984, we reported, by telephone [ Reference (c)], an additional (Item 5) potential 10CFR50.55(e) deficiency regarding Limitorque motor operators. The reportabili y of this item is under evaluation. The following information is provided for the additional Item (5):

Description of Deficiency Item 5 During Startup Testing, three Limitorque motor operators, Model SMB-00/SHB-000, were found to have inoperative torque switches. The problem was traced to a sheared tab on tne phenolic cam of the torque switch. This tab acts as a locking key and when sheared off permits rotation of the torque switch cam without operation of the torque switch electrical contacts.
