Westinghouse W-24 Operational Amplifiers (CDR 86-00-05)
Dear Sir:
On May 1,1986, we reported a 10CFR50.55(e) item to Region I
[ Reference (b)].
The following information is provided under the reporting requirements of 10CFR50.55(e).
Description of Deficiency The deficiency is associated with Westinghouse NAL cards having W-24 amplifiers with date code 8101.
Excessive shift of calibrated trip set points was observed during testing. This deficiency is similar to the deficiency for W-24 amplifiers with date code 8004 which were replaced by Westinghouse in 1983.
Safety Implications There are more than 100 cards in the NSSS and BOP cabinets affected by this problem. These amplifiers are part of the circuitry monitoring system trips. An on-line calibration check of any cards which contain these amplifiers may be required during normal operation.
If the check is performed, the amplifiers may oscillate resulting in set point shift.
