Dear Sir:

l In References (b), (c), and (d), we reported a 10CFR50.55(e) deficiency regarding motor control starters supplied by Gould, Inc., (now Telemecanique).

i Description of Deficiency This deficiency involved the potential for the motor control starters, unitized type NEMA Size 1 and 2, to experience binding or seizure of the contact carrier assembly if the space between the contact carrier post and support housing is less than 0.010 inch.

As stated in Reference (b), a similar deficiency was identified in 1982 and determined to be reportable per 10CFR50.55(e). The corrective action l

taken at that time involved the inspection and modification of starters that had been identified by the manufacturer as p::ssibly having the subject deficiency.

We have completed a 100% reinspection of the subject starters which are l

utilized in Class 1E applications. Our reinspection has identified six Class lE starters with the subject deficiency.

Mr. Richard W. Starostecki Page 2 Safety Implications Failure of a motor starter would prevent the associated safety-related motor from p.cforming its intended safety function.

Corrective Action The manufacturer was notified of the results of our inspection.

Subsequently, the manufacturer issued a revised 10CFR21 notification.

Previous corrective action specified by the vendor was to ensure the 0.010 inch clearance between the contact carrier post and support housing. The revised notification stresses that all subject starters be inspected to ensure i

that washers are installed under the contact carrier cover, in addition to verification of adequate clearance. The manufacturer's explanation for the recurrence of this problem is that previous corrective action did not emphasize the importance of washers installed under the contact carrier cover. Without these washers, the cover screws can be overtightened and cause binding of the contact carrier assembly.

The corrective action for this deficiency involved a 100% reinspection of the subject starters which are utilized in safety-related applications to ensure that washers are installed under the contact carrier cover and that adequate clearance exists between the contact carrier post and support housing.

T'ne corrective action, as specified, has been completed. This is considered our final report on this item.
