Final 10CFR50.55(e) Report: Potential Undetectable Failure in the Engineered Safety Features Actuation System; Corrective Action Complete (CDR-79-00-04)

Dear Sir:

References (b) and (c) discussed a 10CFR50.55(e) deficiency regarding an undetectable failure in the P-4 interlock circuit of the Engineered Safety Features Actuation System (ESFAS). This deficiency concerned the inability to detect a potential failure in the operation of the P-4 inter-lock contacts (auxiliary contacts from the reactor trip and bypass breakers) and interconnecting wiring during the periodic testing of the ESFAS logic.

The corrective action committed to in Reference (c) was to implement the Westinghouse recommended procedures for periodic testing of the P-4 interlock contacts. In our response to RAI 420.74, we committed to in-stalling permanent test switches and meters for implementing these pro-cedures such that entry into the reactor trip switchgear with portable meters was not required.

Since this completes the corrective action for Unit 1, we consider this the final report. The Unit 2 corrective action will be tracked by a Westinghouse Field Change Notice (FCN).
