In reference (b), we reported a 10CFR50.55(e) deficiency regarding manual handwheel operators on safety-related dampers supplied by Pacific Air Products Company. these dampers were furnished with Contromatics actuators.

Each actuator is equipped with a handwheel mounted on a threaded rod with screws into an extended threaded travel stop. This deficiency concerned gauled threads in the extended travel stop, causing the operator and damper to seize. If the operator cnd damper were to seize in a closed or partially closed position, control room habitability may be af fected.

The corrective action, modifications recommended by Pacific Air Products

[ Reference (c)] to preclude operator / damper seizure during normal damper cycling have been completed. As such, this is considered our final report ,

on this item.
