Subj ect : Interim 10CFR50.55(e) Report: Improper Cable Lug Connections in 25 KVA Inverter (CDR 86-00-04)

Dear Sir:

On April 1,1986, we reported a 10CFR50.55(e) deficiency, Reference (b),

regarding potential malfunction of a 25 KVA inverter manufactured by Elgar Corporation due to improper stacking of cable lugs. This deficiency has been evaluated and determined to be reportable under the provisions of 10CFR50.55(e).

Description of Deficiency l

The deficiency is associated with the improper stacking of cable lugs on l the back of the battery input breaker assembly. The electrical connection to the 600A fuse block is incorrectly made via nuts and studs instead of direct connection of the cable lug to the bus which connects to the fuse block. We also found that cable lugs are stacked similarly for AC input power supply connection. Two safety-related inverters are identified as having the improper connections.

Safety Implications The potential ef fect would be abnormal heating at the connections due to high resistance. This could lead to a fault or circuit failure that would render the equipment inoperable.

' Attention: Mr. Richard W. Starostecki Page 2 Corrective Action The cable lugs for DC and AC power supply connections in the af fected 25 KVA inverters will be determinated and properly reterminated in a manner to preclude abnormal heating due to high resistance.

We will inform Region I when the corrective action has been completed.

This letter is being filed pursuant to the interim reporting provisions of 10CFR50.55(e).
