Final 50.55(e) Report: Blocked Air Hose in Vent Lines from Safety-Related Air-Operated Actuators (CDR 85-00-04)

Dear Sir:

/

In Reference (b), we reported a 10CFR50.55(e) deficiency regarding blocked air hose in vent lines from safety-related air-operated actuators.

This deficiency was identified during testing of air-operated valves when the valves did not move or moved slowly in one direction or the other.

The worst case possible would have been the failure of redundant contain-ment isolation valves to close.

The corrective action, as specified in Reference (c), removal of all defective safety-related valve air hoses and replacement with hoses that were properly fabricated or with instrument tubing if the application did not require a hose, has been completed for Unit 1.

As such, this is con-sidered our final report on this item for Unit 1.

Unit 2 hoses have not yet been installed.

Very truly yours, P606190210 860520 w
