Re: Perry Nuclear Power Plant OEPA Permit No. 3IB00016DD Gentlemen, In accordance with Part III, Section 12.C of the Permit, this letter is serving as confirmation of a telephone report of an unauthorized discharge due to a leak in a fire protection system line made to the State Emergency-Response Commission on 1/7/96.

1. The limitation which has been exceeded: No permit limitation has l

been exceeded. This event is being reported under Part III, Section l 12.A.1 "Any noncompliance which may endanger health or the environment". Although the fire protectioa system uses untreated lake water, this leak was not from a normal discharge point from the system and was at a substantial flow rate.

2. The extent of the exceedance: Approximately 96,000 gallons of fire protection system water were discharged to the ground.
3. The cause of the exceedance: A twelve-inch diameter pipe carrying plant fire protection system vater developed a leak.
4. The period of exceedance: From approximately 0315 to 1100 on 1/7/96.
5. If uncorrected, the anticipated time the exceedance is expected to continue: The leak was stopped at 1100 on 1/7/96.

6. Steps being taken to reduce, eliminate and/or prevent recurrence of the exceedance: The leaking line vill be excavated and repaired or replace prior to returning it to service.
