Dear Mr. Engelken:

In accordance with Technical Specifications for Rancho Seco Nuclear Generating Station, Section 6.9.4.2d and Regulatory Guide 1.16, Revision 4, Section C.2.b.4, the Sacramento Municipal Utility District is hereby submitting a thirty-day report of Reportable Occurrence 78-9.

On September 13, 1978, during testing of P-996, Diesel Driven Fire Pump, the engine tripped on overspeed upon starting. An investigation revealed that the engine. to pump coupling joining bolts had sheared. At that time, contrary to Technical Specifications Section 3.14.2.12, only one high pressure pump with a capacity of 2000 gal / min was considered operable.

Immediate corrective action was taken upon discovery by replacing the bolts in the coupling.

This was done as an interim action since the replace-ment bolts were different from those supplied by the manufacturer. An order for the manufacturer supplied coupling bolts was expedited and on September 15, 1978, the temporary coupling bolts were replaced with those supplied by the manufacturer.

At this time a coupling alignment check was performed to assure that the bolt failure was not a result of improper alignment. The alignment proved to be within specifications for the type and size of coupling. As a result, the bolt failure has been attributed to probable "overtorquing" during initial 3

installation. The replacement bolts will be checked to assure that they were not torqued in excess of the values recommended by the manufacturer or i

those values calculated for the particular type and size of bolt.

2 R. H. Engelken October 5, 1978 The District feels that the frequent operation (weekly starts, monthly surveillance tests, etc.) of the pump is adequate to detect any future problems in this area should they arise.

The component involved was a FASTS Coupling, Model 2-1/2B manufactured by Koppers Co., Inc.

There was no plant transient or shutdown associated with this event.
