Dear Mr. Marbet:

In your letter of November 25, 1992, you requested a copy of the Licensee Event Report for the November 9 steam generator tube leak at Trojan, and copies of the documents requested by Robert D. Pollard in his November 23 letter to the NRC.

The LER has not yet been received in Region V. As you know, PGE has thirty days to submit an LER following a reportable event. We will send you a copy of the LER as soon as we receive it.

The package of materials we provided to you and others at the December 1 meeting at Trojan contained all the documents requested by Mr. Pollard, except for the June 16, 1992 memorandum from E.S. Beckjord to Thomas E. Murley. This document merely transmits Dr. Hopenfeld's March 27 write up of his new generic issue. It was inadvertently left out of the packages prepared for the December 1 meeting. A copy is enclosed.

A NEW GENERIC ISSUE: NULT!PLE STEAM GENERATOR TUBE LEAXAGE A Differing Professional Opinion (OPO) was recently filed in the Offica of Research regarding staan generator tube integrity during a postulated main steam line break (MSLB). The OPO apacifically addressed the operation of steam generators with microscopic tube wall cracks. This concem deals with the possibility of having multiple steam generator tube leaks during a MSLB that cannot be isolated. This sequence could lead to cere melt resulting from the loss of all primary system coolant and safety injection fluid in the refueling water storage tank. The DP0 recoemended that plants operating under these conditions have additional sources of borated water readily available to replenish the refueling water storage tank. It was agreed to resolve the DP0 that this concern would be proposed as a new generic issue. This new generic issue is entitled, " Multiple Steta Generator Tube Leakage,' the details of which are enclosed. Since your staff is currently evaluating the issue of microscopic cracks in stata generator tubes, we are submitting this information to you for your review. This is also in accordance with RES Office Letter No.1, Revision 3, ' Procedure for Identification. -

Prioritization, and Tracking of the Resolution of Generic Issues," page 3, second paragraph, which states:

'After their acceptance, generic issues that originate frca

. outside NRR or from an individual within NRR (i.e., not sent through NRR management) will be transmitted by RES to NRR/PMAS for an innedtate action deterstnation and screening for identification of overlap or duplication with already ir. posed or completed Multi-Plant Actions (MPAs). If HRR cannot complete this imediate action detemination and MPA screening within 15 days, RES should be infomed when the NRR review will be completed.'
