EXECUTIVE SUMMARY Enrico Fermi, Unit 2 NRC Inspection Report 50-341/93020 l This inspection included a review of the quality assurance (OA) program. It was an
! announced inspection conducted by a regional reactor inspecto The objective of this inspection was to examine portions of the licensee's QA progra The inspector interviewed staff, reviewed documents and made observations to accomplish this objective. This inspection focused on OA activities performed during 1989 through 1991. Two violations of NRC requirements were identified in this inspection for events that occurred in that period. One violation was for not meeting the procedural requirements and the other was for inadequate corrective action l The following changes were made to the report:

l issue of QA inspector certification was again reviewed in IR 95012. As described l in that report, several deficiencies with the OA inspector certification process were l identified in QA audits in 1989 and 1991. Further review of the audit reports and l discussion with audit personnel revealed that minimal corrective actions were l taken following the 1989 audit findings. However, when the same problems were l discovered during the 1991 audit, the QA auditors correctly escalated the issue and l the certification process was corrected, including bringing in an outside consultant l to recertify the inspectors in question. The inspectors also identified a concern l with the certification and technical ability of one inspector. This was addressed in l IR 96002. The qualifications and training of lead auditors were reviewed by NRC in l irs 93002,94015, and 95002. In all cases, the auditors were found to have the l appropriate training and background prior to being assigned as lead auditors for the l audits reviewe l Conclusions Based upon the inspections listed above, the inspectors concluded that the QA inspectors and auditors were properly trained and certified. While problems with QA inspector certifications existed in 1989 and 1991, the licensee's OA auditors correctly escalated the finding and the licensee took appropriate corrective action solve i l MA.2 Evaluation of ANSI /ASME N510 Qualification and Certification of Testina Personnel l (Formerly Section 3.1)

" Testing of Nuclear Air-Cleaning Systems," for personnel testing high energy particulate (HEPA) filter I Observations and Findinos Three test procedures. 43.404.001, " Division 1 Standby Gas Treatment Filter Performance Test," 43.404.002, " Division 2 Standby Gas Treatment Filter Test,"
l and 43.413.001, " Control Room Emergency Filter Test," required certification to l ANSI N45.2.6, " Qualification of Inspection, Examination and Testing Personnel for l Nuclear Power Plants," for the lead person conducting the tests. Each test l procedure had a signature line for verification that the lead test person was certified l to the above standar l When the three filter tests above were conducted in 1989, the lead test person signature line was signed by two individuals who were not ANSI N45.2.6 certifie l This was identified during a OA Audit, 91-0143. The licensee stated that the tests l were performed under the auspices of a certified contractor, as well as a certified l inspector from the corporate office. However, the QA audit noted that the l signature lines for the above three tests were neither initialed nor signed by either l of the certified individuals. l " Instructions, Procedures, and Drawir.gs" requires that activities affecting quality be l accomplished in accordance with the documented instructions, procedures, or  l l drawings. Failure to comply with the HEPA filter testing procedural requirements, l l ensuring that the lead test person was certified to ANSI N45.2.6, is a violation of l Criterion V (50-341/93020-01(DRS)).
l Quality Assurance management reviewed the security logs and determined that the  .
l certified inspectors were on site and in the test area during periods when the  l l testing was being performed. The licensee also obtained a letter from the  !
l contractor describi.1g the ANSI /ASME N510 on-the-job training conducted for the  '
l individuals. Subsequant HEPA filter tests in 1991 and 1992 were satisfactorily  l l conducted by the same two individuals, indicating an adequate knowledge level for l performing HEPA filter testing. The licensee removed qualification to ANSI N45. l as a requirement for HEPA filter testing, following confirmation from the ASME l code committee that qualification to ANSI N45.2.6 was not required. Therefore the corrective action to the violation was considered adequate and no response is require l Although certification to ANSI N45.2.6 was not required, qualification to l ANSI /ASME N510-1980 was necessary. Section 4.3 of ANSI /ASME N510-1980, l Page C-4, stated, " Tests shall be made only by persons who have demonstrated their competence to satisfactorily make the specific tests in question, as evidenced by experience and training." The two individuals who signed off as lead test l personnel for the 1989 HEPA filter tests had not received training on ANSI /ASME l N510. This was also identified by the licensee's QA auditors in audit 91-014 l DER 91-0589 was generated which identified the lack of training and certification l of the lead test personne As part of the corrective action to DER PM589, the licensee developed a training course for ANSI /ASME N510. Howeve.r, the licensee exempted three lead test l personnel, including the two who had signed off on the 1989 tests, principally l because they participated in subsequent HEPA filter testing. The inspectors also l noted that, as of December 1996, the licensee had not taught the cours l Furthermore, The waivers made no reference to training conducted for these l individuals. According to Fermi interfacing Procedure FIP-TOI 16-SO Revuion 2, requests for waivers "shall be supported by certified or authenticated documents such as official transcripts, verifiable certificates of completion, etc." No such l documentation was identified to justify waiving the training requirement. 10 CFR l Part 50 Appendix B, Criterion XVI " Corrective Action" requires that significant l conditions adverse to quality are promptly and adequately corrected. Therefore, l waiving the training requirement in response to the DER appeared to constitute an inadequate corrective action. This is considered a violation (50-341/93020-02(DRS)).
l The licensee presented the inspectors with plant records, including a statement l from the certified contractor indicating the on-the-job training performed prior to l and during the 1989 testing. Based on these records, the inspectors determined l that, although the three lead test personnel did not receive formal training on l ANSI /ASME N510, the on-the-job training appeared to be adequate. l l l The inspector reviewed two 1990 DERs during the original 50-341/93020  l l inspection. Additional DERs were reviewed during subsequent inspection i l
i l Observations and Findinos    i l DER 90-310 documented the failure to write a DER for a potential missed hold l point. During audit 90-0125, an auditor identified that a QA inspection hold point l on a weld appeared to have been missed. However the DER written on this issue l focused on the fact that no DER was written rather than on the missed hold poin l The inspectors determined that the originator of the weld process control sheet had l evidently mistakenly entered the QA inspection nold point by the step for pre-fitup l cleanliness rather than by the step for weld fitup, as was normally done. At the time the work was done, the QA inspector realized the error, and performed the l necessary inspection during the fitup step, rather than as specified on the weld l process sheet. The QA inspector, however, failed to document the change made to l the inspection hold point. The licensee's corrective actions to the DER focused on h ensuring that weld hold points were properly specified and witnessed in the futur These corrective actions were determined to be acceptable and the DER was closed in July 199 l DER 90-324 documented a missed QA verification (yellow lining a drawing) of a l valve counterweight removal. Since the yellow lining had not taken place, the QA l auditor was concerned that the counterweight could not be confirmed as having l been removed. The auditor also expressed concern that the inspector was l ' pressured to sign the package due to scheduler concerns. As the counterweight l was physically present outside the drywell, the licensee originally considered that to l be sufficient proof of its removal. However, tais was not adequately documented l either in the original work package or during closure of the DER. Due to the l inadequate documentation, it was questioned whether the counterweight was truly l removed. The DER was closed with a confirmatory corrective action, to enter the l drywell and look at the valve in question, transferred to a work request. This work l request was later canceled due to the actuator being replaced with a different typ l l The licensee first performed an engineering analysis to show why the actuator l could not still be attached to the valve, and when that proved to be insufficient to l resolve the issue, the licensee entered the drywell during a refueling outage and j expended dose to take pictures of the valve from all angles to verify that the .

Several DERs (90-310,  l l 90-324, and 93-363) were written related to the adequacy of the yellow lining !

The inspector reviewed various past inspection reports and examined available l documents pertaining to the fire protection program, especially in regard to ths l reporting chain for the fire protection program. The inspectors determined that l there had been some minor problems associated with the licensee's fire protection l program in the mid 1980's. Both the licensee and the NRC Region til staff were l aware of these issues and they were corrected. The fire protection program is l subject to routine NRC inspection. The most recent fire protection inspection was l conducted in June 1995. As documented in inspection Report 95009, the fire l protection program was considered to be excellent at that time. When inspection Report 50-341/93020(DRS) was corrected and issued on February 21, 1997, we inadvertantly failed to correct some erroneous dates. These incorrect dates were brought to our attention and have now been corrected. Correction of the dates was determined to have no effect upon the previously established conclusions of the report.

Please insert the attached sheets into your copy of the corrected Inspection Report i 50-341/93020(DRS). We regret any inconvenience this may have caused you.
