SUBJECT: FERMI INSPECTION REPORT 50-341/99003(DRP)

Dear Mr. Gipson:

On April 1,1999, the NRC completed an inspection at your Fermi 2 reactor facility. The

enclosed report presents the results of that inspection.

During this inspection period, we noted that the operation of your facility was conducted in a 1

professional and safety-conscious manner. Specifically, we observed prompt and appropriate

response to the increase in off-gas radiation levels by your staff. We also observed appropriate

supervisory oversight of maintenance and surveillance testing activities. In addition, we noted

that you implemented effective radiological control measures and performed these activities in

accordance with approved work instructions.

We are concerned with the continuing operator errors during routine activities. One operator

error of note involved the failure to follow a surveillance test procedure which caused an

emergency diesel generator output breaker to trip open due to a reverse power condition.

Another operator error involved the improper operation of the centrifuge feed tank agitator which

! caused an inadvertent transfer of radioactive resins and resulted in two members of your staff

receiving a small unexpected dose. Although your staff responded appropriately to prevent an

l overexposure event, the casual factors which resulted in this occurrence appear to be a

recurring problem.

Based on the results of this inspection, the NRC has determined that two violations of NRC

requirements occurred. These violations are being treated as Non-Cited Violations (NCVs),

consistent with Appendix C of the Enforcement Policy. These NCVs are described in the

subject inspection report. If you contest the violation or severity level of these NCVs, you should

provide a response within 30 days of the date of this inspection report, with the basis for your

denial, to the Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington DC

20555-0001, with copies to the Regional Administrator, Region lil, and the Director, Office of

Enforcement, United States Nuclear Regulatory Commission, Washington, DC 20555-0001. i
