On June 8,1998, the NRC completed an inspection at your Braidwood Units 1 and 2 reactor

facilities. The enclosed report presents the results of that inspection. During the 7 weeks

,

covered by this inspection period, your conduct of activities was generally characterized by

safety-conscious operations, sound engineering and maintenance practices, and careful

radiological work controls.

We are concemed about a failure to take adequate corrective measures for a condition

adverse to quality. Specifically, three non-environmentally qualified components were found in a

safety-related motor control center located in an area subject to a harsh environment in the event

of an accident. This violation of equipment qualification requirements was determined to be due

to a failure to initiate a review of non-environmentally qualified components used in safety-related

applications as part of the corrective measures for an earlier violation identified in inspection

Report 50-456/97012.

We are also concemed about two examples of a failure to follow station administrative

procedures which resulted in a violation for the failure of maintenance personnel to de-energize

the power to a welding machine and to bleed off the pressure from the oxygen and acetylene

hoses of an oxygen-acetylene torch. Failure to follow procedures has been highlighted in

previous inspecticn reports and appears to be a recurring problem.

In addition, we are concemed about the frequency of mispositioned equipment. Six

non-safety-related valves were mispositioned in May. The number of mispositioned pieces of

equipment on a monthly basis has shown an increasing trend in 1998. Although the events this '

year mostly involved non-safety-related equipment and have not resulted in an event, the

increasing trend is note: 'orthy and warrants consideration.

The violations are cited in the enclosed Notice of Violation (Notice), and the circumstances l

surrounding the violations are described in detail in the enclosed report. Please note that you are

l- required to respond to this letter and should follow the instructions specified in the enclosed
