Dr. J. Nelson Grace SPR 86-009 June 30, 1986 ENCLOSURE 1 EVENT DESCRIPTION On June 16, 1986, RMA-14, the Noble Gas Effluent Monitor on the Reactor Building Purge Supply and Exhaust System, was declared inoperable due to spiking above the high radiation setpoint with no detectable radiation present.

ACTION TAKEN The channel was not returned to an operable status in 72 hours8.333333e-4 days
0.02 hours
1.190476e-4 weeks
2.7396e-5 months
; therefore, the preplanned alternate method of monitoring was implemented.

The cause of the inoperability was determined to be an adjustment problem in the timing circuit of the auxiliary circuit board. The necessary adjustments were made and the channel was returned to service on June 29, 1986.
