VIRGlt C. SUMMEP. NUCLEAR STATION DOCKET NO. 50/395 OPERATING LICENSE NO. NPF-12 SPECIAL REPORT (SPR 910005, Suppicment 1)
South Carolina Electric & Gas Company submitted Special Report 91-005 en September 16, 1991, documenting the Main Plant Vent High flange Process Monitor, RMA-13, being inoperable due to spiking of the monitor. At the time of submittal, the cause cf the spiking had not been identified and it was noted that a supplemental report would be subinitted when the unit was repaired and returned to service.
The cause of the spiking has been attributed to tha introduction of noise via a grour.d loop. Troubleshooting determined that the signal cable shield wire had been grounded at several points which created the ground loop.
This problem has been corrected by grounding the field cable at one point.
The unit was tested satisfactorily for 96 hours0.00111 days <br />0.0267 hours <br />1.587302e-4 weeks <br />3.6528e-5 months <br /> and then declared operable on October 16, 1991. The specific wiring drawings will be revised to provide clearer guidance on shield grounding. This action will be completed by March 31, 1992.
