Dear Mr. Ebneter:

This Special Report is submitted pursuant to the requirements of Technical Specification 3.7.10, " Fire Rated Assemblies."

On May 17, 1989, fire door 18-203 was declared inoperable due to broken welds and warping of the door. The subject door is located in a high traffic area and the damage is contributed to the door being struck at the base with carts loaded with various items. A replacement door has been requisitioned and it has been placed on the " Critical Items List." As of this date, no shipment date for the replacement door has been received. However, upon arrival of the replacement door, priority will be given for the installation and testing. In addition, the Licensee is presently pursuing installation of

" kick plates" on fire doors located in high traffic areas.

On May 28, 1989, the Licensee shut down the reactor and entered a mini-outage for repairs associated with the pressurizer safety valves. To facilitate entry into the Reactor Building, fire doors RB-303 and PA-301 were opened.

Each of the doors remained open for the duration of the mini-outage. Fire door PA-301 was returned to service June 7, 1989, and RB-303 was returned to service June 9, 1989.

For each of the events identified, compensatory action was taken as required by the Action Statement of Technical Specification 3.7.10.
