Dear Dr. Grace:

This Special Report is submitted pursuant to the requirements of Technical Specifications Section 3.7.9, "Fire Suppression System," and Section 6.9.2.

On June 26, 1988, during scheduled surveillance testing, the Diesel Fire Pump failed to start in response to a valid test signal. Recorders were installed at various points to monitor the starting circuitry but repeatability of the failure did not occur. Visual inspection of the control circuit panel identified that some connectors were not seated firmly.

The control panel was removed and taken to the maintenance shop. All connectors were reworked to achieve a positive connection, wiring terminations were verified tight, and the interior was thoroughly cleaned.

The unit was reinstalled and starting of the pump was performed locally, remotely, and by low pressure actuation. All testing was satisfactory and the unit was declared operable July 10, 1988.
