.D APPENDIX A NOTICE OF VIOLATION Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Generating Station License No. NPF-57 As a result of the inspection conducted on March 10, 1987 - April 13, 1987, and in accordance with " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C (Enforcement Policy 1986), the following violation was identified:

A.

Technical Specification 2.2.1 requires that the main steam line radiation high-high trip setpoint be established at a value less than or equal to 3.0 times full power background. With a setpoint less conservative than 3.6 times full power background, the affected channel shall be declared inoperable.

Technical Specification 3.3.1 states that, with less than 2 operable main steam line radiation channels per reactor protection trip system, at least one trip system shall be placed in the tripped condition, and the plant shall be placed in STARTUP with the main steam isolation valves closed within 6 hours6.944444e-5 days <br />0.00167 hours <br />9.920635e-6 weeks <br />2.283e-6 months <br /> or in at least HOT SHUTDOWN within 12 hours1.388889e-4 days <br />0.00333 hours <br />1.984127e-5 weeks <br />4.566e-6 months <br />.

Contrary to the above, on April 10, 1987, main steam line radiation high-high trip setpoints for channels A, B, C, and D were found to be set less conservative than 3.6 10mes full power background. These setpoints had been placed in effect during November 1986.

Since November 1986,the unit has been operated at full power for extended periods without taking the action required by Technical Specifications 2.2.1 and 3.3.1.

This is a Severity Level IV violation (Supplement 1).

Pursuant to the provisions of 10 CFR 2.201, Public Service Electric and Gas Company is hereby required to submit to this office within 30 days of the date of the letter which transmitted this Notice, a written statement or. explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be 1taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
