This letter transmits the findings of the routine, resident safety

inspection conducted by Mr. G. Meyer, Mr. D. Allsopp, and others from July
'

12, 1988 to August 29, 1988 at the Hope Creek Nuclear Generating Station.
The findings were discussed with Mr. J. Hagan of your staff.

During the inspection period, rou;ine testing revealed that the wrong
!
valve had been rendered inoperable during a system tag-out and this same
!
valve was not reactivated upon removal of the tag-out.
This inadequate
l
control of equipment alignment is cited in the Notice of Violation
enclosed herewith as Appendix A.
You are required to respond to this
letter and in preparing your response, you should follow the instruction
,
j
in Appendix A.

Our inspectors found that problems existed in your actions to resolve
previously identified concerns with the control of scaffolding.
It is

important that identified problems be promptly and properly corrected.
i
You should review the implemented corrective actions in this area to

assure their effectiveness.

This letter also acknowledges receipt of your August 12, 1988 response
,
to the violation concerning contractor employee discrimination cited in
I
the Notice of Violation issued July 14, 1988. As discussed in the enclosed
-
,
report, those corrective actions were found to be acceptable.

l
Our inspectors reviewed two licensee identified violations concerning
!
locked high radiation doors found unlocked and omitted sample analyses on
i
!
a liquid effluent release. As discussed in the enclosed report, no
l
citations were issued for these violations,
j
