Dear Sir:

This report is submitted in accordance with Section 6.7.2.B.3 of the Technical. Specifications for Cooper Nuclear Station and discusses a reportable occurrence that was discovered on October 16, 1978. A licensee event report form is-also enclosed.

Identification of Occurrence:

An observed inadequacy in the implementation of. administrative and procedural controls which could have reduced the degree of redund-ancy provided in an Engineering Safety System.

Conditions Prior to Occurrence:

Reactor was at ~ steady state power level of approximately 87% of rated thermal power.

Description of Occurrence:

. Cable HP 58, Division I, was found routed through Division II riser under Control Room panel 9-3. l Designation of Apparent Cause of Occurrence:

- Construction error in not following scheduled routing.

' Analysis of Occurrence:

The cable HP 58, Division I, is used for control and indication of HPCI valve 49MV (M015). The cable, No. 12 AWG with 600V insul-ation, is used in a circuit of 125V DC. It runs with Division II cables for a distance of about 3 feet. This type of cable is' qual-ified to meet requirements specified in CNS original construction contract.

The possibility of cable failure or fire between the two division trays is remote. The Cable Spreading Room is provided with sprink-1er system. (A heat sensor for sprinkler system initiation is located near these trays.)

Corrective Action:

Cable HP 58 will be rerouted through the correct Division I riser during the next shutdown of sufficient duration.
