4.0 RESPONSIBILITIES Ebasco Services, incorporated Is responsible for supplying all 4.1 materials, receiving Inspection, testing and documentation of such tests are required to support J. A. Jones Construction Company and their Subcontractors in performing their work.

J. A. Jones Construci ton Company and their Subcontractors are 4.2 responsible for forming, placing aggregate, grouting and curing of coment pressure grouted area, the epoxy pressuring grouting for sealing cracks and the surface preparation and topping and finishing of top surface as required by this procedure.

J. A. Jones Quality Verification is responsible for Inspection 4.3 and documentation the activities of J. A. Jones Construction Company and their subcontractors covered by Reference.3 1.

50 INSPECTION J. A. Jones Quality Verification Personnel shall perform inspection 499502-10B as necessary to assure of repair activities on Placement compilance with all applicable requirements of Reference 31.

Cored holes covered by this checklist are 7, 9, 14, 15, 18, 19, 21, 22, 23, and 24.

All of the above holes shall be cleaned of all foreign material and free 2.

W-SITP-15 Attachment B Inspection Checklist for REPAIR OF TOP SURFACE 1.

J. A. Jones Construction Company Engineering sha*1 map the surface of the entire pour delineating the areas that can be made to meet the required elevation and finish by grinding and the areas that must be chipped out to sound concrete and topped.

2.

The areas requiring chipping and topping shall be chipped.down to a minimum of one and one-half Inch (li") below required elevation.

3.

Af ter chipping to sound concrete and prior to any topping or grinding of another area, J. A. Jones Construction Company Engineering shall notify Ebasco Engineering area is ready for inspection.

4.

Any area suspected of containing unsound concrete shall be inspected by the Windsor Probe method.

In verifying the concrete soundness, the equivalent average 28 day compressi.ve strength shall not be less than 5000 PSI and none of'the equivalent compressive strengths shall be less than 4600 PSI.

5.

Areas requiring grinding only to produce an acceptable surf ace shall be ground with suitable grinding machines.

6.

Prlor to epoxy application, verify that shelf, life of epoxy has not expf red.

7.

The surface of the chipped out area shall be cleaned and free from standing water, then coated with a neat coat of Sika HI-Mod Epoxy mixed and applied per manufacturer's requirements.

8.

Af ter neat coat of epoxy, place 4000 PSI Class AA Concrete using pea gravel as coarse aggregate t.o required elevation and finish as required by approved construction drawings.

9 After the required finish has been obtained, cure repair per requirements of W-SP-7

W-SITP-15 Attachment C Inspection Checklist for WATERSTOP EMBEDMENT 1.

Area to be repaired shall be chipped to sound concrete, dry and free of dust, laitance, grease, and other foreign material.

2.

Waterstop shall be straightened and supported as required.

3 Epoxy shall be Sikadur HI-Mod Gel.

4.

Aggregate shall be Colma Quartzt te Aggregate.

5.

Mixing shall be in accordance with manufacturer's Instructions.

6.

Application of prepared epoxy mortar to embed waterstop shall be per Appendix A, W-SP-7 7.

Mortar shall be applied in layers no greater than one (1) Inch in thickness.

8.

If required mortar thickness exceeds one Inch (1), allow preceeding layer to cool before applying second layer.

9.

Cooling should take approximately one (1) hour.

10. When more than one (1) layer is required,' a neat coat of Sikadur HI-Mod spoxy to preceeding layer before applying additional layers of mortar is not required,

l REPAIR OF VERTICAL CORED HOLES WITHOUT CRACKS l

Cored holes to be covered by this procedure are numbers 1, 2, 3, 4, 5, 6, 7, 8, 11, 12, 13, 16, 17, and 20.

2.

Holes shall be thoroughly clean and free from standing water.

3.

Inside surface of hole and surface of area chipped out to expose reinfore-ing steel shall be coated with neat Sikadur Hi-Mod Epoxy.

4.

Mixing and application of epoxy shall be in accordance with manufacturer's recommendations.

5.

Prior to use, verify that shelf life of epoxy has not expired.

After coating with epoxy, holes shall be filled in layers with 4000 PSI R-2 Class AA Concrete.

Assure that all work is done in strict accordance with applicable procedures.

2.

An Inspection Checklist shall be completed for each inspector for each shif t.

3.

In the event that the work inspected by one inspector on one shift is less that the total work covered by a checklist, the inspected shall clearly state the extend of work inspected in the " Comments" section.

1.

Both north and south voids shall be chipped to sound concreto. All voids shall be completely clean.

2.

Prior to application, verify that shelf life of apoxy has'not expired.

3.

Verify all surfaces of existing concrete in both north and south large void has been coated with Sika-Dur Hi Mod epoxy which has been mixed and applied in accordance with manufacturar's recommandations prior to the placing of fresh concrete.

4.

North and south large voids are to be formed in accordance with paragraphs l

9.2 and 9.3 of W-SP-7 respectively.

5.

Concrete shall be placed in accordance with W-SP-7 paragraph 9.2 for l

the large north void, and paragraph 9.3 for the large south void.

6.

Concrete placed in large voids shall be adequately vibrated and consoli-l 1

dated.

Smaller voids shall be repaired by coating surface with Sika-Dur Hi Mod epoxy mixed and applied per manuf acturer's recommendations then dry packed per Ebasco Specification LOU-1564.472,Section II.

Repair shall be cured by leaving forms in place. Areas not formed cad the top surface shall be cured with burlap and water per U-SP-7.

After curing has been completed on north and south void, forms shall be removed and waterproofing applied over entire West race of placement 105 l

per J. A. Jones' Work Procedure, W-WP-11, " Waterproofing".

An additional twelve inch (12") wide layer shall be installed over the exposed edges of the first layer.

All exposed edges shall have a crowelled bead of mastic applied after the membrano installation is complete. Terminate bottom and top of waterproofing in reglets per paragraph 9.7 of W-SP-7.

2.

All of the above holes shall be cleaned of all, foreign material and free of all standing water, before the grouting operation is started.

3 Pressure gage must be calibrated and certified.

4.

Pea gravel to be used must be in sealed bags.

5.

Expanding plug, as shown in Appendix 0, W-SP-7, shall be available for each hole.

6.

Grouting operation shall begin with Hole #24.

7.

Epoxy grout shall be mixed and applied in accordance with manufacturer's re commendations.

8.

Prlor to use, verify that shelf Ilfe of epoxy has not expired.

9.

Hole #24 packed with pea gravel from sealed bag.

10.

Begin pressure grout operation on Hole #24.

11.

Continue pressure grout application until epoxy is observed passing into one of adjacent core holes.

12. When epoxy is observed in adjacent hole, maintain pressure for additional 30 minutes or until excessive grout appears *In adjacent cored holes.
13. Place pea gravel into hole in which epoxy was observed flowing and begin grouting operation.

14.

Continue above procedure until all holes with connecting cracks have been grouted.

15 Holes without connecting cracks shall be grouted as outlined above.

16. Hole number 22 shall be grouted, as above, except when grout is observed passing through the connecting crack into the South vold on the West side.

The pressure shall be relleved and the opening of th's crack plugged with Sika Set Plug then the pressure resumed and grouting continued until all air is removed and grout is forced out the vent.

.s W-SITP-15 Attachment E Inspection Checklist for PRESSURE GROUTING VOIDS AND HORIZONTAL CORED HOLE #25 IN WEST FACE 1.

Both North and South void shall be chipped to sound concrete.

2.

Voids shall be completely clean and dewatered.

3 Both voids shall be formed in accordance with W-SP-7 4.

Voids shall be packed with aggregate and wet down.

5.

Vent and grout pipes shall be installed in accordance with W-SP-7 6.

Liquid portland cement grout shall be applied through " slip" pipe until grout is forced out through bo,ttom grout nozzle and top vent.

7 When grout is observed entering hole drilled down froa top surface, hole shall be filled with aggregate.

8.

When grout is forced out of all vents, " slip" pipe shall be removed while under pressure.

9.

When " slip" pipe is removed, grout shall be applied through bottom grout nozzle until grout is forced through all vents.

10.

Repair shall be cured.by leaving forms in place..

11. Horizontal cored hole shall be repaired'in the same manner as the volds referenced above.
12. After curing has been completed on North and South void, forms shall be removed and waterproofing applied over entire West Face of placement 10B per J. A. Jones ' Work Procedure, W-WP-11, " Waterproofing". An additional dwelve Inch (12") wide layer shall be installed over the exposed edges of the first layer. All. exposed edges shall have a trowelled bead of mastic applied after the membrane installation is complete.

W-SITP-15 Attachment A Inspection Checklist for EP0XY PRESSURE GROUTING CORED HOLES WITH CRACKS 1.

Cored holes covered by this checklist are 7, 14, 15, 18, 19, 21, 22, 23,

'ard 24.

2.

All of the above holes shall be cleaned of all, foreign material and free of all standing water, before the grouting operation is started.

3 Pressure gage must be calibrated and certified.

4.

Pea gravel to be used must be in sealed bags.

5 Expanding plug, as shown in Appendix D, W-SP-7, shall be available for each hole.

6.

Grouting operation shall begin with Hole #24.

7 Epoxy grout shall be mixed and applied in accordance with manufacturer's recommendations.

8.

Prior to use, verify that shelf life of epoxy has not expired.

9 Hole #24 packed with pea gravel from sealed bag.

10.

Begin pressure grout operation on Hole #24.

11.

Continue pressure grout application until epoxy is observed passing into one of adjacent core holes.

12. When epoxy is observed in adjacent hole, maintain pressure for additional 30 minutes or until excessive grout appears in adjacent cored holes.

13 Place pea gravel into hole in which epoxy was observed flowing and begin grouting operation.

14.

Continue above procedure until all holes with connecting cracks have been grouted.

15 Holes without connecting cracks shall be grouted as outlined above.

16. Hole number 22 shall be grouted, as above, except,when grout is observed passing through the connecting crack into the South void on the West side.

The pressure shall be relieved and the opening of the crack plugged with Sika Set Plug then the pressure resumed and grouting continued until all air is removed and grout is forced out the vent.

W-SITP-15 Attachment B inspection Checklist for REPAIR OF TOP SURFACE i

1.

J. A. Jones Construction Company Engineering shall map the surface of the entire pour delineating the areas that can be made to meet the required elevation and finish by grinding and the areas that must be chipped out to sound concrete and topped.

2.

The areas requiring chipping and topping shall be chipped down to a minimum of one and one-half inch (li") below required elevation.

3.

After chipping to sound concrete and prior.to any topping or grinding of another area, J. A. Jones Construction Company Engineering shall notify Ebasco Engineering area is ready for Inspection.

4. - Any area suspected of containing unsound concrete shall be inspected by the Windsor Probe method.

In verifying the concrete soundness, the equivalent j

average 28 day compressive strength shall not be less than 5000 PSI and none of the equivalent compressive strengths shall be less than 4600 PSI.

Areas requiring grinding only to produce an acceptable surface shall be ground with suitable grinding machines.

6.

Prior to epoxy application, verify that shelf life of epoxy has not expired.

7 The surface of the chipped out area shall be c. leaned and free f rom standing water, then coated with a neat coat of Sika Hi-Mod Epoxy mixed and applied per man'ufacturer's requirements.

8.

After neat coat of epoxy, place 4000 PSI Class AA Concrete using pea gravel as coarse aggregate to required elevation and finish as required by approved construction drawings.

WATERSTOP EMBEDMENT i

Area to be repaired shall be chipped to sound concrete, dry and free of dust, laitance, grease, and other foreign material.

2.

Waterstop shall be straightened and supported as required.

3.

Epoxy shall be Sikadur Hi-Mod Gel.

4.

Aggregate shall.be Colma Quartzite Aggregate.

5 Mixing shall be in accordance with manufacturer's instructions.

Mortar shall be applied in layers no greater than one (1) inch in thickness.

If required mortar thickness exceeds one Inch (1), allow preceeding layer to cool before applying second layer.

When more than one (1) layer is required, a neat coat of Sikadur Hi-Mod epoxy to preceeding layer before applying additional layers of mortar is not required.

W-SITP-15 Attachment E inspection Checklist fo r PRESSURE GROUTING VOIDS AND HORIZONTAL CORED HOLE #25 IN WEST FACE 1.

Both North and South void shall be chipped to sound concrete.

2.

Voids shall be completely clean and dewatered.

3 Both volds shall be formed in accordance with W-SP-7 4.

Volds shall be packed with aggregate and wet down.

5 Vent and grout pipes shall be installed in accordance with W-SP-7,

6. - Liquid portland cement grout shall be applied through " slip" pipe until grout is forced out through bottom grout nozzle and top vent.

7 When grout is observed entering hole drilled down from top surface, hole shall be filled with aggregate.

8.

When g' rout is forced out of all vents, " slip" pipe shall be removed while under pressure.

9 When " slip" pipe is removed, grout shall be applied through bottom grout nozzle until grout is forced through all vents.

10.

Repair shall be cured by leaving forms in place.

11. Horizontal cored hole shall be repaired in the same manner as the volds referenced above.
12. Af ter curing has been completed on North and South void, forms shall be removed and waterproofing applied over entire West Face of placement 108 per J. A. Jones ' Work Procedure, W-WP-ll, " Waterproofing". An additional twelve inch (12") wide layer shall be installed over the exposed edges of the first layer. All exposed edges shall have a trowelled bead of mastic applied af ter the membrane installation is complete.
