5.0 QUALIFICATION REQUIl IENTS:

shall be determined by a suitable evaluation of the candidate's education, experience, training, test results or capability demonstration.
Initial qualification or upgrading requirements shall be determined for a candidate on a case by case basis by the Q.A.
Manager, with input from the candidate's immediate superviscr.

provisions that follow are to be used as a guide in determining justi-I fication for qualificaction or upgrading.
Each qualification or

High school graduation plus ten years of related experience in equivalent inspection, examination, or testing activities, or high school graduation plus eight years experience in equivalent inspection, examination, or testing activities, with at least two years as Level II, and with at least sufficient training to be acquainted with the relevant quality assurance aspecks of a nuclear facility, or -

Completion of college level work leading to an Associate Degree and seven years of related experience 6

5.2 Required Capabilities for Inspection and Testing Personnel:

Each person who performs inspections and tests shall be physically capable of performing the assigned 4
task.
5.2.2 Technical

Level I kersonnel Capabilities l
A Level I person shall be capable of performing the inspections, examinations, and tests that are required 4
to be performed in accordance with documented procedur.es.
The individual shall be familiar with the tools and equipment to be employed and shall have demonstrated j
proficiency in their use.
The individual shall also be i
capable of determining that the calibration status of inspection and measuring equipment is current, that l
the measuring and test equipment is'in proper condition for use, and that the inspection, examination, and test procedures are approved.

Level II Personnel Capabilities l
A Level II person shall have all of the capabilities of?

as appropriate; in supervisien or maintaining surveillance over the inspections, examinations, and test; in super-vising and certifying lower level personnel; in reporting inspection, examination, and testing results; and in the evaluating the validity and acceptability of inspection, examination, and test results.

Level III Personnel Cacabilities A Level III person shall have all of the:; capabilities of a Level II person for the inspection, examination or test category or class in question.
In addition, the individual shall also be capable of ev.aluating the adequacy of specific programs used to train and test inspection, examination and test personnel whose qualifications are covered by this procedure.
6.0 TRAINING
6.1 Personnel considered for Level I o; II certification shall receive appropriate training and indoctrination to the Mercury Company requirements under the direction of a Level III individual.
6.2 Th wit date for certifi to as satisfactory understanding of the requirements and methods for the applicable inspection or test method and to determine individual training requirements.
7.0 CERTIFICATION OF Q"ALIFICATION:
7.1 The qualifications of all inspection and test personnel shall be documented on Form 253, " Qualification of Inspection and Test Personnel Certification Record", Exhibit 9.2.
7.2 A file of records of personnel qualifications shall be established for each individual containing, as applicable, records of past performance history, training, certification records, eye examination reports,'and other pertinent data which supports the basis used for certification.
8.0 EVALUATION OF PERFORMANCE:
8.1 The Job performance of inspection and testing personnel shall be evaluated at periodic intervals not to exceed three years.
The evaluation shall be to determiret the continued proficiency of the individual and shall be documented on Form 253.
If it is determined that the capabilities of an individual are not in accordance with the qualifications specified for the job, that person shall be removed from activity, until such time that the required capability has been demonstrated (see 6.2 above).
