1.1 To delineate the method by which approved design changes and field revisions are incorporated into the Contract Documents and implemented.
2.0 SCOPE 2.1 This procedure applies to all design changes and field revisions affecting Contract W3-NY-11.

DESIGN CHANGES AND FIELD REVISIONS 4.8 Field Revision - A modification made to the design documents by the Company as authorized by the Contract. A field revision does not change the design intent and will allow installation to con-tinue without a Design Change.
5.0 RESPONSIBILITY 5.1 It shall be the responsibility of the Project Engineer or his designee to incorporate approved design changes the Contract Documents.
5.2 A design change may be requested by the Company if, in the opinion of the Project Engineer, such action is warranted. It shall be the responsibility of the Project Engineer of his designee to request a design change via submittal of an Information Request (IR) to the Engineer.
5.3 Only Ebasco Services may approve design changes.
5.4 It shall'be the responsibility of the Project Engineer or his designee to made field revisions as authorized by the contract and ASP-IV-37.
The Project Engineer or his designet shall perform interference checks prior to making field revisions.
6.0 INSTRUCTIONS 6.1 Design Changes 6.1.1 Requesting a Design Change - a design change may be requested by the Company by submittal an IR to the Engineer.

6.1.1.1 A log shall be kept of submittal and answered IR's. Blank IR forms shall be pre-numbered consecutively. A copy of the sub-mitted IR shall be replaced with a copy of the answered IR when received. This log shall be kept by the Project Engineer.

DESIGN CHANGES AND FIELD REVISIONS Page of 6.1.1.2 The person initiating the IR shall clearly state the proble: requiring a design change and shall also include a reconnended design change when appropriate. A T-B Department Supervisor should review and approve the IR prior to submittal to the Engineer to avoid duplications.
6.1.1.3 In the case of a discrepant or nonconforming situation, an IR shall not be initiated prior :o the completion of the appropriate steps as set forth in TBP-12.
6.1.1.4 A copy of all answered IR's shall be distri-buted to the QA Supervisor for review.
6.1.2 Type of Design Changes - An approved design change is received from the Engineer in one of the following forms:
A. FCR - A Field Change Request is a design change originated by the Engineer's field office.

B. DCN - A Design Change Notice is a design l
change originated by the Engineer's home office (New York).
C. Revised Drawing or Specification.
D. Redlining per ASP-IV-37.

6.1.3 Receipt and Distribution of Design Changes-The 're'ceipt and distribution of design changes shall be as outlined in TBP-1, Document Control Procedure. The Q'A Supervisor shall be included on the distribution of all design l
changes.

"F" of ate DESIGN CHANGES AND FIELD REVISIONS A. FCR, DCN - Upon receipt of the FCR orDCN, the affected contract document (s) shall be stamped with a change notice stamp and the FCR or DCN number recorded thereon, with a notation indicating whether it is a DCN or FCR as detailed in TBP-1.
In addition, it shall be perr.issible to mark up the affected document in red showing the changes made.
All marked up drawings shall be initialed and dated by the person making the change.
B. Revised Document - Upon receipt of the revised docu-ment, the voided document shall be replaced with the revised document. The voided document shall then be disposed of per TBP-1.
C. Redlining - As decialed in ASP-IV-37.
6.1.5 Implementation of the Design Change 6.1.5.1 Design changes affecting categories 1 through 5 piping and safety related and Seismic 1 pipe supporting assemblies shall be implemented via revision of the asso-ciated traveler as follows.
A. FCR, DCN, - the traveler drawing copy shall be. marked up as stated in Paragraph 6.1.4 (A) and a copy of the FCR or DCN should be included with the traveler as additional information. The traveler shall be revised to depict the changes made.
B. Revised Document - the superseded
. traveler drawing copy shall be voided per TBF-1 and the revised drawing shall be inserted to depict the changes made.

- 9' 6.1.5.2 Design changes affecting categories 6 thru 8 piping and nonsafety, non-seis=ic pipe sup-porting asse=blies shall be i=plemented via revision to the field copy of the associated drawing or specification. This revision shall be accomplished by replacing the voided field document with the revised docu-ment or, in the case of an FCR or DCN, by marking the applicable change on the field document, or by redlining.
6.1.6 FCR,DCN log - Logs shall be kept of all FCR's and DCN's by the Project Engineer or designee. The logs shall indicate when the FCR or DCN was received, when it war incorporated in the Tompkins-Beckwith Engineering File Drawing Copy, and when it was incorporated into the field drawing copy (or when the traveler was revised in the case of category 1 through 5 piping and safety related, seismic 1 pipe supporting assemblies).
6.2 Field Revisions 6.2.1 Field revisions shall be controlled as follows:
A. Field revisions shall be made only by members of the Company Engineering Department who are authorized by the Project Engineer. Field revisions shall be made only as authorized by the Contract and will follow the guidelines and limitations of Ebasco Procedure ASP-IV-37.
B. The required field revision shall be denoted in, red on the field copy of the design document and ini-tialed and. dated by the person making the revision prior to implementation.
C. The field copy of the design docunent shall be stamped "AS BUILT BY FIELD", " INSTALLED PER AS-BUILT", or " INSTALLED PER DESIGN" (See Samples).

NOTES: 1. All field revisions affecting welding of categories 1, 2 and 3 piping and safety related, Seismic 1 pipe supporting assemblies shall require revisions per TBP-35 (piping) or TBP-24 (supports).
- 2. Field revisions which affect welding on cate-gories 4 and 5 piping shall require revisions per TBP-35.
D. The T-B Engineering file copy of class 1, 2 and 3 piping ISOS shall also be " red-lined" to agree with field copy within 48 hours5.555556e-4 days <br />0.0133 hours <br />7.936508e-5 weeks <br />1.8264e-5 months <br />.
E. Upon completion and acceptance of the installation, the field copy of the design document which has been
" red-lined" shall be signed and dated by the Aree Engineer verifying that the installation conforms to the dssign document incleding field revisions.
F. A copy of the " red-lined" document showing all revisions shell be transmitted to the Engineer upon completion of the installation.
NOTE: DELETED G. The Engineer shall perform an' evaluation of the As-Built Information.

If the as-built conditica is acceptable, the
" red-lined" document shall be marked approved by the Engineer and returned to the Company. The approved as-built document shall be maintain,ed as a QA record until the as-built information is incorpocated into a revised drawing or until turnover of such records to the Engineer.
B. If the as-built condition is unacceptable the Engineer shall notify the Company regarding a suitable resolution. Such resolution shall either be new design information or direction regarding an acceptable change. In the latter case, steps A through G shall be repeated.

7 of 8 Page DESIGN CHANGES AND FIELD PIVISIONS C. For Piping Systems which are identified as 200*F and above operating temperature, the Engineer must perform a stress analysis.
6.2.2 A log of all field revisions shall be kept by the Proj ect Engineer or designee. The log shall identify the design document affected by the field revision and include the date when information regarding the field revision was transmitted to the Engineer.
6.2.3 When revisec documents are received from the Engineer.
they shall be checked by the Project Engineer against the as-built documentation to verify that field revi-sions are accurately reflected on the revised design

document. When a field revision has been included on a revised document, a notation shall be made in the field revision log.
6.2.4 Before code stamping, in the care of work cevered by ASME,Section III, the field revision documentation shall be reviewed to assure that all applicable field revisionr have been incorporated into the revised drawings.
6.3 All "Per Design" drawings shall be stamped and signed and dated by the area engineering representative.
6.4 Deleted.
F 6.5 Final as-builts - Final as-built drawings shall confor= on Table 1.
7.0 AUDITS 7.1 Implementation of this procedure shall be audited by Quality.
