1.0 SCOPE 1.1 This document describes the qualifications, certification and duties of individuals performing Quality Assurance and Quality Control functions for Coating Service Level I of nuclear facilities.

5.1.8 Vendor Inspector.

5.1.8.1 High school graduate and two years college or technical school, plus one year experience in coatings.

5.1.8.2 one year high school chemistry 'or one semester college level chemistry.

5.1.9 The education and experience requirements specified above need not be considered absolute when other factors (such as demonstrated capability in a given job through previous performance or satisfac-tory completion of proficiency testing) provide reasonab1'e assur-ance that a person can completely perform the required tasks.

5.1.10 The sline QC Training Course is required for certification as either a.14 vel I, IAVel II, or Vendor Inspector.

5.1.10.1 ne Quality Assurance Manager shan be responsible for the training curriculum used.

5.1.10.2 The training program shall be administered by either the Quality Assurance Manager or the Site Quality control Mans-ger.

5.1.10 3 The results of the training shall be documented on the

" Employee Training Record Form (Form SQA1-1-3) and main-tained by the Quality Assurance Manager.

5.2 Documentation and. certification..

5.2.1 A file on each individual's qualifications 'shall be established and maintained at both home office and job site.

5 2.1.1 This file shan contain records of past performance, work history, training, qualification, and certification records.

5.2.1.2 Information on each individual shall be summarized on the "QA/QC Certification Summer,y". Form SQA1-1-2.

c 5.2.2 A "QA/QC Certification" Form SQA1-1-1 shall be completed for each individual certified. The form shan be maintained in the indivi-dual's file and shall indicate the fonowing.

5.2.2.1 The Employers name.

5.2.2.2 The individual's name.

5.2.2.3 Activity qualified to perform.

(2) 5.2.3 2 1he Quality Assurance Manager shall be authorized to certi-fy the QA Engineer, Site Quality Control Manager. Quality Control Supervisors and Inspectors, and the Vendor Inspec-tor.

5.2.4 Certification shal1 be valid for two years; at that time the indi-vidual must be recertified by the same requirements which then apply to certification.
