One example dealt with a deficiency report being written stating:
Item (1) - Support and anchor installation locations do not agree Item (2) - Two anchor locations do not agree (22' between COL's)
F&M responded on October 3, 1983, that:
Item (1) - Traceability can be established Item (2) - Verified and corrected On October 7, 1983, the Ebasco QA Surveillance Group conducted field verification of this support and noted that the elevation still did not match and that the actual installation is 3-3/8" anchor bolts instead of 2-3/8" anchor bolts as shown on F&M's QC checklist.

Another example dealt with a support being inspected after it was supposedly removed and this inspection report is now in so called
" dead file."
The documentation contained several other examples similar to the ones shown above that have been identified by Ebasco QA IRG.
