SUBJECT:

ADDITIONAL INFORMATION CONCERNING THE ALLEGATION INVOLVING EBASCO'S QA REVIEW 0F FISCHBACH AND MOORE (F&M) RECORDS FOR SEISMIC SUPPORT INSTALLATIONS o ok ie informed the NRC inspectors that he has been terminated by Ebasco.

He supplied the NRC inspectors with an interoffice memo (attached) concerning the interpretation of ANSI N45.2.6 which he does not believe is correct. He said that in his experience a Level I inspector's work is always directly observed by a Level II. He also said that the procedures dealing with testing of anchor bolts have been routinely violated.

than expanding their sample size as required. He left this new address:

In essence a Level II or III must review the results as reported and documented by a Level I inspector. This is in the form of a signature by a Level II or III on the inspection documentation. ANSI N45.2.6 Goes not require reinspection of a Level I's work by a Level II or III.
