m AUDIT 1.0 SCOPE 1.1 This procedure describes the Internal Audit System to be used where GEO Construction Testing is performing nondestructive examination (NDE) services.
1.2 Radiation Safety Audits are not included in this pro-cedure.
i 2.0 PURPOSE 2.1 To verify conformance to, and implementation of, the total Quality Assurance Plan.
2.2 To establish a numerical rating of the project for comparison 'to previous'and subsequent addits' to' deter-

mine improvement or deterioration in the conformance to the Quality Assurance Plan.
3.0 RESPONSIBILITY 3.1 The Quality Assurance Manager is respons ible for the implementation of this procedure.
3.2 Personnel performing Quality Control Audits shall be responsible to the Quality Assurance / Quality Control Departments.
This responsibility shall be either as a direct assignment to said departments or may be granted as a specific assignment by the Quality Assur-ance Department head.
In any case, no person having direct responsibility to an audited system may be used to audit that system.
4.0 REFERENCE - Quality Assurance Program, Section 18.
5.0 GENERAL INFORMATION AND INSTRUCTIONS 5.1 Audits are the means by which management is kept in-formed of the adequacy of the implementation of, and compliance to, the Quality Assurance Plan.

In the event the Auditor and Project Manager cannot

reach a mutual agreement regarding the audit findings and/or recommended corrective actions, the matter shall be resolved by the Quality Review Board.
The findings of the Quality Review Board shall be final and made a part of the audit report.
Note:
The Quality Review Board shall consist of:
a) the Auditor; b) the Project Manager; c) the Quality Assurance or Quality Control Manager;

6.9 Any audit finding (s) requires a written response with-in 30 days.from receipt.
The response shall state the
.results of the review and the extent of any investiga-tion, the corrective actions taken and any actions taken to prevent recurrence.
Objective evidence of the above shall be included in the audit response, as ap-plicable.

6.9.1 The response shall be prepared and signed by the Pro-H ject Manager, or a designee approved by the Project Manager, and submitted to the Quality Control Manager.
6.9.2 In the event the necessary corrective and/or preven-tive actions cannot be completed by scheduled comple-tion date, the reasons why and the new completion date shall be requested.
Request and concurrence shall be documented and f orm a part of the audit.
Concurrence is by the Quality Control Manager, request is by the Project Manager.
6.9.3 A follow-up report shall be provided listing the re-quirements of Paragraphs 6.9 and 6.9.1.
0.10 The-Quality Centrol Manager shall be responsible for:

m AUDIT 6.2 Audit Checklist 6.2.1 Inte rnal audits shall be performed in accordance with an " Audit Checklist".
All elements of the Quality As-surance plan will be audited.
6.2.2 All of the checklist shall be completely filled out and signed by both the Auditor and the Project Man-ager.
6.2.3 If for some reason a particular segment of the audit checklist does not apply to the project or cannot be performed, the. reason shall be noted on the appropri-ate segment.
6.2.4 Each item of the audit has been as. signed a.per.centage point value'.
These points are granted if t' hat speci-fic item meets the minimum requirements of the Quality Assurance Plan.
No points are allowed for discrepant areas.
6.2.5 The total' value of percentage points for each segment
~
of the audit shall be noted in the appropriate space on the cover sheet.
These totals shall be averaged to obtain the final quality rating of the project. -
6.5 Audit findings shall be ' reviewed with the Project Man-ager prior to final preparation of reports.
6.4 One (1) copy of the audit checklist and one (1) copy of the audit report ( Exhibit 1-Form 5.1-1, Typical) l will be given to the Project Manager.
The original shall be submitted to Corporate Quality Control for review by Executive Management and the Quality Assur-ance Manager.
6.5 Copies -of audits and related documentation shall be on file at the GEO Construction Testing project facility and in the Quality Control Department.
6.6.
Audit results shall be documente d on a " Quality Con-trol Audit / Inspection" report form.
The Auditor shall address the report to the Project Manager and include the following information, as a minimum.
