9/12/79 1.0 Purpose This procedure describes the general requirement for the qualification of inspection personnel.

.2.0 Scope The requirements of this procedure are intended to qualify inspection personnel to perform the inspections required to assure acceptability of dome construction activities in accordance with the referenced standard.

These inspections include the verification of formwork, reinforcing steel placement, and concrete placement including finishing and curing as de-fined in the project Construction Procedures.

4.0 Definitions None 5.0 Requirements 5.1 The requirement for qualification of personnel contained in this procedure are intended to meet the proficiency requirements neces-sary to perform inspections to the specific requirements of the project Construction Procedures.

5.2 Personnel certified to this. procedure shall have appropriate know-ledge of required inspection practices and procedures and shall have demonstrated sufficient proficiency in the methods, techniques and requirements applicable to their work assignment and certifica-tion level.

5.3 Personnel performing inspections, examinations or tests shall be capable of performing the physical demands of the project. Natural or corrected near-distance acuity shall be such that the applicant is capable of reading J-1 letters on a standard Jaeger test chart or equivalent.

5.4 Eye tests shall be performed on a twelve month cycle and a certifica-tion of acceptable performance shall be included in the applicant's qualification record.

5.5 Personnel implementing the inspection requirement as defined in the Construction Procedures (CP's) shall be qualified as Level I and shall be under the direct supervision of a Level II.

9/12/79 5.6 Personnel' evaluating or reporting inspection on test results shall be qualified as Level II.

5.7 Personnel certifying Level I and Level II inspectors shall.be quali-fied to Level III requirements as defined in paragraph 6.2.

6.0 Oualification Level Recuirements 6.1 Level I 6.1.1 A Level I inspector shall,have sufficient experience or training to perform the inspections defined in the appropriate CP's. _This experience or training shall assure that:

The inspector has been indoctrinated in the require-

ment of the procedures defining the activity.

6.1.2 Training or indoetrination may be provided to supplement or substitute for experience when required. Training may be provided by in-house training sessions, available commercial programs or on-the-job under the direction of an appropriately qualified inspector. Training /indoc-trination shall be provided in the following general areas:

Preplacement inspection including joints 3.

Placement practices 4.

Curing, finishing and repair inspections 5.

Forming and reinforcing steel inspection 6.1.3 Where training or indoctrination is provided to supplement or substitute-for experience, the program shall consist of a minimum of six (6) hours and shall be directed at the specific requirements contained in CP's and QAP's established for the project. Records of this indoctrination /

training shall be entered on the qualification worksheet maintained in the project contract files.

6.1.4 A Level I inspector shall be a high school graduate appro-priately trained on the job or possess a minimum of one year's experience in related inspection activities.

A Level II inspector shall have experi~ence and train-ing in the performance of required inspections and tests and in the organization and evaluation of the results of the inspections and tests. He shall be indoctrinat'ed in the specific Construction Procedures appropriate to the project.

6.2.2 A Level II inspector shall be capable of supervising or maintaining surveillance over the inspections and tests performed by others and of calibrating or establishing validity of calibration of inspection and measuring equipment.

t 6.2.3 A Level II inspector shall have demonstrated profi-

_ciency in planning and setting up of inspections and test and shall be capable of determining the validity of results.

6.2.4 Level II personnel shall satisfy one of the following requirements:

1.

Graduate of a four year accredited engineering or science college or university, plus two years of experience in quality assurance including testing or inspection (or both) of equivalent construction and installation activities.

2.

High school graduate, plus four years of experi-ence in testing or inspection (or both) of power plant, nuclear plant, heavy industrial, or other similar equipment or facilities.

6.2.5 The education and experience requirements specified in paragraph 6.2.4 may be modified when other factors, such as demonstrated capability or previous performance in a similar project operation, are known and provide

reasonable assurance that the individual can competently perform a particular task.

6.2.6 Where training or indoctrination is provided to sup-plement or substitute for experience, the program shall consist of a minimum of twelve (12) hours and shall be directed at the specific requirements contained in CP's and QAP's established for the project. Records of this indoctrination / training shall be entered on the qualifica-tion worksheet maintained in the project contract files.

9/12/79 6.3 Level III 6.3.1 Level III personnel shall have b?oad, experience and training in the performance of inspection and test activities and shall possess comprehensive knowledge in the principles and techniques of the inspections to be performed. They shall be capable of planning and supervising inspections and tests, reviewing and approving procedures and evaluat-ing the adequacy of activities to accomplish objectives.

They shall be capable of organizing and reporting results and of certifying the validity of results.

i 6.3.2 Level III personnel shall possess the following educational background:

1.

Graduate of a four-year accredited engineering or science college or university, plus five years of experience in quality assurance, in-cluding testing or inspection (or both) of equivalent manufacturing, construction and installation activities. At least two years of this experience should be associated with nuclear facilities; or if not,the individual shall have training sufficient to acquaint him thoroughly with the safety aspects of a nuclear facility.

High school graduate, plus~ ten years of experi-ence in general quality assurance or engineer-ing of equivalent manuf acturing, construction and installation activities. Five years of this experience is required in quality assur-ance, including testing or inspection (or both) of-equivalent manuf acturing, construction and installation activities. At least two years of this experience should be associated with nuclear facilities; or if not, the individual shall have training sufficient to acquaint him thoroughly with the safety aspects of a nuclear facility.

6.3.3 The corporate Level III shall be designated by the President of FPS in a memorandum. The memorandum shall define the method of certification based on the requirements of this procedure.

7.0 Personnel Certification 7.1 Having success fully satisfied the appropriate experience, education or training / indoctrination requirement, the candidate shall be issued a qualification certificate valid for a two year period approved by a certified Level III. This shall be in memorandum form and shall list the certification limitations.

9/12/79 7.2 Requalification at the end of the two year period may be granted based on continuing acceptable performance or other requirements deemed appropriate by the Level III.

.7.3 The qualification certification shall include the following items:

Signature of Employer's Designated Representative 7.4 The corporate Quality Assurance Manager shall be responsible for the administration and maintenance of quality personnel certifica-tion records. He shall assure that copies of certification records are maintained in the site quality and project contract files.

7.4.1 Project contract files shall contain, as a minimum, the following records; as applicable:
