2.0 Purpose The purpose of this procedure is to establish and describe the method for performing the review of documentation supporting the qualification of

l inspectors used in the performance of NDE testing.

3.0 Scope This task shall include a review of qualification documentation of 1

personnel of the testing contractors. The task shall be accomplished by l

verifying the qualifications are in accordance with prescribed requirements.

4.0 Responsibilities 4.1 Task Leader As directed by the LP&L Quality Assurance Manager, the Task Leader will coordinate activities, provide support, and compile data for l

submitting to the Quality Assurance Manager.

5.0 Procedure 1

5.1 Auditor will review the qualification records of a minimum of twenty (20) per cent of the personnel involved in NDE activities. The documented review will review the qualification records of the NDE personnel to assure compliance with the stated requirements. A checklist will be prepared, based upon stated requirements, to be

used to document review of records.

5.2 Deficiencies noted will be processed as outlined in QP 19.1, Task Force Installation Verification.

If a new employee has had pre-vious certification with another company, he may be allowed to test for an equivalent level in similar applications or methods, providing the following conditions are met to the satisfaction of the Quality Assurance or Control Manager.

Evidence of prior certification, including history of training and experience.

The employee was working in the capacity to which he has been certified within six (6) months prior to his termination.

Reference 1, paragraph 6.7.1 states as follows:

Each' Level I and II examinations shall consist of a general (written), a specific (written) and a practical (demonstration) section.

I A visual acuity examination shall be administered to the candidate prior to ate initial certifi-4 cation, and thereafter, on an annual basis or at more frequent intervals, if required. This examination.shall be performed by a professional optometrist, medical doctor,' nurse or other qualified persons. The candidata shall have natural or corrected near distance acuity in at least one (1) eye such that he is capable of reading a minian= of J-1 letters on a standard Jaeger test chart at a distance of not less than 12 inches. The ability to perceive an Ortho-Racer minimum of 10, or similar test pattern, shall be acceptable.

After the candidate has completed the qualifi-cation examinations in the allotted time, the Level III NDE Examiner shall sign the exami-nation form in the appropriate place and submit the candidate's NDE experience resume', NDE armninations and visual acuity examination to the Quality Control Manager for grading and approval.

21. Reference 2, paragraph 4.5.2 states as follows:

The above examination requirements for Level III Technician may be waived when Peabody Testing Services elects to certify a Level III with a written statement providing the individual meets the minimum requirements of Table 3-3.

In such cases, the certification shall indicate that certification was established without er==ina-tion. When requested Peabody Testing Services shall furnish a written statement verifying the qualifi:ations of any CNT Level III individual.

Upon approval, the Manager of Quality Control shall issue a certification card showing method, level of certification. (for Assistant Techni-clans the certification card will list specific procedures) and re-certification due date.
