Site audits shall be conducted primarily by LP&L Site Quality Assurance

storing, cleaning, erecting, installing, inspecting, testing, operating, maintaining, repairing, and modifying safety related equipment, struc-l

[4.0 Responsibilities

4.1 Quality Assurance Engineers and Technicians are responsible for conducting site audits, for preparing a report of their audit findings and for follow-up in the form of reaudits on corrective action cournitments.

4.2 The LP&L Project Quality Assurance Engineer is responsible for review of site audit reports to maintain awareness of site activi-i ties and of site problem areas.

4.3 Specialists, consultants, or other knowledgeable personnel shall be utilized for technical support in audits where deemed necessary by the Quality Assurance Manager and/or the Project Quality Assurance Engineer.

5.2.2 Unscheduled Audits i

Checklists or audit plans shall be used to ensure depth and continuity of the audit. The checklist shall be used as a guide and shall not restrict the audit investigation when I

findings raise further questions that are not specifically included in the checklist.

Nonconformances or program deficiencies shall be identified and investigated further to identify the cause and effect and i

to determine the extent of corrective action required.

Nonconformances or def!.ciencies shall be related to the require-ments which have been violated.

Elements fotmd to be nonconforming shall be discussed with a member of the audited organization. He shall be requested I

Conditions requiring immediate corrective action shall be I

reported (verbally) insnediately to management of the audited organization. A written report in accordance with Section 7.0 below shall be prepared following the verbal contact.

7.1 Preparation of the Audit Report I

i The assigned auditing Quality Assurance Engineer / Technician shall prepare the audit report. The report shall be made using the

" Site Audit Report" form LPL Q-21 (See Figure 18.3-1.).

i 7.2 The Project Quality Assurance Engineer will issue the audit report

reaudit, a Continuation Sheet of the Site Aud.t Report shall be used.

The Continuation Sheet (LPL Q-22) shall indicate the status of the item established during subsequent audits along with any further recemenda-tions by the auditor and provide a maana of response by the audited or-ganization. (See Figure 18.3-3).

6.3 Surveillance schedule 6.3.1 Precaration TheProjectQAEngineershallprepareAnAuditSchedulefor the next one month period, listing the subject of each audit to be performed. The Audit Schedule, Figure 18.4-1, shall be used to tabulate the audit activity details. The Project QA Engineer shall periodically review the audit schedule and revise as necessary to assure that coverage and schedule reflect current activities or unusual conditions. The following

6.4.1 Updating Within 15 days after completion of the scheduled period, the Project QA Engineer shall update the audit schedule by. noting j

the actual date of the audit and the date of the issuance of the Audit. Report. When the response to the audit has been received, this shall be noted on the audit schedule. When

. corrective action has been confirmed, this shall also be noted on the schedule. The " Remarks" section can be used to note any n'ecessary special considerations. The updated Audit Schedule

shall become a summary history of the audit activity during the schedule period.

7.0 Records l
