Mr. Ja me s P. C'Reilly SPR 84-006 Page Two May 25, 1984 EVENT DESCRIPTION On April 26, 1984, while performing a surveillance of fire barriers in the Intermediate Building (412 elevation), degraded Kaowool used for electrical fire barriers was noted on one (1) cable tray and four (4) conduits.
Maintenance Work Requests (MWR) were submitted, the fire detection system was verified operable, and a one hour roving fire watch was established as required by Technical Specification 3.7.10.
Repair work was completed the following day, and the roving fire watch was secured.
The results of a reinspection of the repair by the Fire Protection Coordinator (FPC), conducted on May 4, 1984, were unsatisfactory.
All of the degraded Kaowool had not been repaired.
Immediate corrective action was taken to repair the Kaowool, and the one hour roving fire watch was reinitiated.
Repair and a satisfactory reinspection was performed on May 5, 1984.
CAUSE OF THE EVENT The cause of this event was due to inadequate documentation of the requested repair work and the failure of personnel involved to adequately communicate job status.
ACTION TAKEN Damaged or degraded fire barriers will be identified by placing a Maintenance Item Identification tag at each unsatisfactory location.
If several tags are placed, the total number of tags i
will be identified on the associated MWR.
When the repair work is complete and appropriate QC inspections performed, the MWR will be forwarded to the FPC so that reinspection can be performed.
After reinspection, the FPC will forward the MWR to the Shift Supervisor for disposition and removal from the Removal and Reinstallation Log and the Roving Fire Watch Log.
