Date/ Time Event April 1, 0001 Unit 2 in Mode 5. Low Pressure Safety Injection pump 2P016 on Shutdown Cooling, approximately 3600 gpm.

RCS level 6 3/4" above flange, fill and vent in progress. RCS boron concentration is 1875 ppm.

Boric Acid Makeup discharge line to FV0210V clogged; RCS emergency boration paths are via gravity feed lines.

April 19, 0425 Entered Mode 4.

April 25,1340 Entered Mode 5 due to time requirements of Tech.

Spec. 3.4.5.2. Reactor Coolant System check valve leak rate unsatisfactory.

April 27, 2047 Entered Mode 4 to retest RCS check valve leakage.

April 28, 2150 RCS inventory balance surveillance, S023-3.37, unsatisfactory at 12.7 gpm. Unidentified leakage in excess of 1 gpm.

April 28, 2230 Unusual Event declared at Unit 2.

April 29,1448 Terminated Unusual Event. RCS leakage reduced to acceptable limits.

April 29,1545 Initiated plant shutdown to Mode 5 due to excessive '

RCS pressure boundary isolation valve measured leak-age.

April 30, 0140 Entered Mode 5. RCS <200 F.

April 30, 2359 Plant in Mode 5; temperature 124 F; RCS depressur-ized: pressurizer level is at 10%.
