At 1300 on March 2, 1992, the 4B Emergency Diesel Generator (EDG) was declared inoperable due to an inadequate surveillance of valve SV-3434B,. which was installed during the Dual Unit Outage. During a review of surveillances performed to meet Inservice Test (IST) require-.

ments, it was discovered that the measurement of stroke time of valve SV-3434B was not performed within the surveillance period. The valve exercise test was performed on January 30, 1992; however, the valve's stroke time was not measured at that time. As this valve controls flow to the 4B EDG day tank, valve SV-3434B and EDG 4B were declared out of service. The valve surveillance was completed satisfactorily at 1630I, and both valve SV-3434B and the 4B EDG were returned to service at that time. The root cause of the event was personnel error, in that not all IST requirements for valve SV-3434B were incorporated into plant procedures. Surveillance procedures will be revised to ensure that al'1~

IST requirements for valve SV-3434B are incorporated. The IST program will be reviewed to assure that each test or exam for each IST compo-nent is adequately identified in the surveillance tracking system. IST are required by Technical Specifications. A missed 'urveillances Technical Specification surveillance is reportable under 10 CFR 50.73 (a) (2) (i) (B) .

I. EVENT DESCRIPTION At 1300 on'arch 2, 1992, with both Units 3 and. 4 in Mode 1, the 4B Emergency Diesel Generator (EDG) (EK, EIIS:DG) was declared inoper-able due to an inadequate surveillance of valve SV-3434B.

During a review of surveillances performed to meet Inservice Test (IST) requirements, it was discovered that the measurement of stroke time of valve SV-3434B was not performed within the, surveil-lance period; The survei'llance was due.to be performed on January 30, 1992, with the grace period expiring, on February 21, '1992. The valve exercise test was performed on January 30, 1992, howeve'r, the valve's stroke time was not measured at. that time..

Upon, determining that the January 30, 1992 surveillance did not meet the requirements of .,the IST program, valve SV-3434B was declared out of service (OOS). As this valve may isolate flow to the 4B EDG day tank,. EDG. 4B was also declared OOS. The valve surveillance was completed satisfactorily at 1630, and'oth valve SV-3434B and the 4B EDG were,retu'rned to service .at that time.

IST surveillances .are required by Technical Specifications. A missed Technical Specification surveillance is reportable under

10. CFR '50.73(a) (2) (i) (B) .

II. EVENT CAUSE Valve SV-3434B was installed during the dual unit outage,(DUO)I which was'ompleted in October, 1991. During, the DUO, two new safety grade EDGs were instal'led at Turkey Point. Valve SV-3434B is a self-enclosed. solenoid valve, having neither position indica-tion. nor a control switch for actuation. Based on IST requirements of similar valves at Turkey Point, it was believed that exercise testing without a timed stroke test would meet IST requirements.

The procedural changes for performing valve'exercise,testing were completed prior to the 'end of the DUO. In December 1991, determined'hat the measurement, of stroke time was also it was necessary to meet IST requirements. Florida Power and 'Light 'Company (FPL) subsequently submitted .several relief requests on December 31, 1991. One of these relief requests described the IST testing which was to be performed on valve SV-3434B. On January 24, 1992, the NRC granted interim relief for all reliefs submitted in December, 1991.", Both the relief request and,the interim relief explicitly stated that stroke time testing was to be performed: quarterly for, valve SV-3434B.

The root cause of the event was personnel error, in. that not all IST requirements- for valve SV-3434B were incorporated into plant procedures. Contributing causes were failures to identify and track changes to the IST program which .were necessitated by the instal'lation of valve SV-3434B.

.from the Diesel Oi'1 Transfer Pump on high. Day Tank level. The March 2, 1992, surveillance of valve SV-3434B demonstrated the va'lve's continued operability from,the time of its installation.

Based on the above, the health arid safety of the public was not affected.

CORRECTIVE ACTIONS

1. Surveillance procedures will be revised to ensure that all IST requirements for valve SV-3434B are incorporated. This action will be completed by April 15, 1992.

2'. All interim reliefs granted on January 24, 1992, will be reviewed to assure that the appropriate test requirements have been incorporated into the surveillance procedures.. This action will be completed by April 15, 1992.
