Between December 2 and December 4, 1991, two 20-foot Sea-Land containers were loaded with contaminated scaffolding. Several inspections of the containers were performed prior to loading to identify defects which could compromise the containers integrity. No leakage was observed during any inspection. On December 5, 1991, this shipment was released and consigned to the Quadrex Recycle Center in Oak Ridge, Tennessee.

On December 6, 1991, at 0908, Health Physics (HP) was notified by the consignee that a small amount of liquid had leaked onto the ground where the vehicle was parked, in Jasper, Florida. HP requested the Quadrex Recycle Center to immediately dispatch their radiological emergency response team to take radiological measurements and report their findings to FPL, and to control the leak and clean up any contamination found. The leaking fluid was determined to be uncontaminated water. Condensation was observed in the container, and there was evidence that some water had escaped through the lower door seal. This event occurred because the lower door seal of the container did not prevent leakage of condensation. The condensation developed as the temperature of the air around the container dropped from over 80'F at Turkey Point, to less than 40 F in Jasp'er. Door seals will be caulked with a flexible silicon sealant. Containers that are already filled and ready for shipment will be unloaded, inspected, and reloaded.

TURKEY POINT UNIT 3 50-250 91-013-00 02 oF 03 EVENT DESCRIPTION Between December 2 and December 4, 1991, two 20-foot Sea-Land containers, numbers QG32 and QG12C, were loaded with contaminated scaffolding in accordance with procedure O-HPS-041.4, "Packaging Low Specific Activity Radioactive Waste for Shipment to Waste Processors." Sea-Land containers are large,'end-loading metal boxes commonly used for this purpose. Before being filled, these containers were inspected for defects such as cracks, dents, punctures, damaged gasket material, or other items which could compromise their integrity. They were inspected again by Quality Control and Health Physics (HP) personnel after being filled, but before being loaded onto a semi-tractor trailer for shipment. A final inspection was conducted by a Florida Department of Health and Rehabilitative Services (DHRS), Office of Radiation Control, inspector prior to release. No leakage was observed during any inspection.

On December 5, 1991, at approximately 1900, radioactive shipment number 91-79, consisting of containers QG32 and QG12C, was released from Turkey Point, consigned to the Quadrex Recycle Center in Oak Ridge, Tennessee.

On December 6, 1991, at 0908, HP was notified by the consignee that a small amount of liquid, described as an oily, watery substance, had leaked onto the ground from container QG32, at the Fuel City Truck Stop in Jasper, Florida, where the vehicle was parked for the driver's mandatory rest period. HP requested the Quadrex Recycle Center to immediately dispatch their radiological emergency response team form Gainesville, Florida, to Jasper; to take radiological measurements and report their findings to FPL; and to control the leak and clean up any contamination found. The Plant Supervisor Nuclear was notified at 0915. At about, 0945, authorities from Hamilton County, where Jasper is located, informed HP that they had secured the area around the vehicle. The State of Florida DHRS, Office of Radiation Control was notified at 1025; they dispatched a team of Health Physicists to Jasper from their Jacksonville office.. An FPL team was assembled and dispatched to Jasper from Turkey Point, consisting of two Radiation Protection Men, the cognizant engineer, and the Corporate Health Physicist.

Each of the teams dispatched to Jasper surveyed the leak location and found no contamination.

With permission from DHRS, HP opened the door of the leaking container to ascertain the source of the leak. Condensation was observed clinging to the ceiling of the container, and there was evidence that some water had run down the side of the container to the floor and escaped through the lower door seal. The door was closed and sealed with RTV sealant. DHRS released the vehicle and the shipment was returned to Turkey Point, escorted by a Radiation Protection Man. The shipment arrived at Turkey Point at 0315 on December 7, 1991.

This is a voluntary LER submitted in accordance with the guidance of NUREG 1022, Revision l.

TURKEY POINT UNIT 3 50-250 91-013-00 03 QF 03 II. EVENT CAUSE The cause of this event was an inadequate lower door seal on container QG32. The seal did not prevent leakage of condensation which developed as the temperature of the air around the container dropped from over 80'F at Turkey Point, to less than 40'F in Jasper.

III'VENT SAFETY ANALYS1S The leaking fluid was identified as uncontaminated water. Based on the above, this event did not affect the health and safety of the public.

IV+ CORRECTIVE ACTIONS Sea-Land container shipments were suspended immediately.

2. Procedure O-HPS-041.4 will be revised as follows:,
a. Sea-Land containers will be lined with absorbent sheeting or waterproof sheeting material with a granular absorbent, protected with plywood as necessary, to trap condensation.
b. Door seals will be caulked with a flexible silicon sealant such as RTV.

The revisions will be completed by December 15, 1991.

3. The eleven containers on site that are already filled and ready for shipment will be unloaded, inspected, and reloaded in accordance with the revised procedure. This will be completed by March 31, 1992.
