Safety-Related Heating, Ventilating, and Air Conditioning (HVAC) Systems Procedure Review The NRC inspector reviewed Standard Site Procedure (SSP) 38, Revision 1, " Installation and Inspection of HVAC Duct / Supports and Accessories," and Bechtel specification No. SV279VS1003, Revision 6,

" Installation of Safety Class and Non-Safety Class HVAC Equipment I

and Ductwork." Review of the procedure and specification requirements indicate that the installation of HVAC Ductwork, seismic supports,

, and other safety-related aspects of the installation are reviewed and l approved by appropriate AE/ licensee personnel.

The NRC inspector verified that approved procedures and instructions were adequate for proper installation and inspection of the safety-related HVAC systems. The NRC inspector also verified that controls were established to ensure the following: (1) that the type and classification of seismic supports for the HVAC ductwork complies with approved drawings, (2) provisions for required pre-installation and in process inspections are performed at the appropriate time for HVAC safety-related equipment, (3) the location of equipment, dampers, and filters is correct, (4) bolts, nuts, washers, gasket material and sealants are of the proper type, correctly installed, and (5) where required, bolt preloading (torquing), minimum bolt embedment, and thread engagement criteria were adequat The NRC inspector reviewed two additional specifications in conjunction with the HVAC specification to determine item No. 4 abov During the specifications review there was one item which appeared to be misleading in specification SV279VS1003, paragraph 3. Discussions with QC and craft personnel indicated that the specification was not clear on the application of Dow Corning 999 on steel-to-concrete surfaces, also, the manufacturers data sheet does not recommend Dow 999 for concrete applications. In discussions with licensee management personnel it was stated that Dow Corning 999 will not be used in concrete applications and the specification will be changed to reflect this. Overall, the procedures and specifications reviewed appeared to be adequat No violations or deviations were identifie b. Observation of Work

, The NRC inspector examined portions of safety-related HVAC systems.

! Portions of the system observed included ducts, supports, fans, l gasket material, valves, plenums, and air handling units. HEPA filters were not observed because they have not been installed. HEPA filters will be installed when the system has been balanced. During

this inspection there were two discrepancies noted which were:

l (1) Support 1-6-0128-S095 was installed with a 2x2x1/4 inch angle instead of a 2 1/2 x 2 1/2 x 1/4 inch angle as required by the installation drawing (2) The use of cadmium or electroplated zinc nuts on hot dipped galvanized bolts was observe This is in conflict with the l specification.

The licensee / contractor perfor:r.cd surveillances for both types of discrepancies in other areas. For discrepancy (1), the l licensee / contractor selected a sample of approximately 66 supports

for reinspection to assure compliance to the Engineer's design. No discrepancies were found. Discrepancy (2), the licensee / contractor performed a surveillance which covered duct work located in Unit 1

'MEAB, FH8, and RCB. A total of 193 duct connections and access doors Jwere examined during the surveillance. There was an average of 16 bolts per connection for a total of 3088 bolt and nut combination Of these connections, only 4,were found to be in question, totaling 7 bolt / nut combinations. The results of this surveillance indicated that the intermingling. problem is within the acceptance level of the Bechtel. multiple sample plan (95/95).

Based on the results of the surveillances performed, these-two discrepancies will be considered as isolated case Also during this inspection the NRC inspector and an HVAC test supervisor observed some air leakage around a conduit pipe leading into the fan housing on Exhaust Fan No. 3V111VFN01 This HVAC system is the exhaust line from the Battery Roo FCR No. BH-01984 was initiated to track and correct the problem. This will not be considered as a violation by the NRC inspector because the test supervisor discovered the leakage. The NRC inspector requested the licensee to examine other similar fans for the same problem and an

' additional five fans were included on the FCR No. BH-0198 The safety-related HVAC systems inspected are as follows:

' The NRC inspector reviewed record packages of HVAC duct sections, fan motors, dampers, and supports. The traveler packages reviewed contained all required documents necessary to examine the items including the current FCR's. NCR's were also in the traveler packages for deficiencies noted during QC inspections. Review of these NCR's and FCR's indicated that they were approved and signed by appropriate personnel and that the response, corrective action, and-changes proposed were adequat .The NRC inspector reviewed one audit report and three general surveillance reports concerning the installation of safety-related HVAC and supports. The required audits and surveillance reports had s been performed in accordance with licensee's schedule. Audit

findings are considered to have been reported in sufficient detail to l-permit a meaningful assessment for appropriate correc'.ive action and
final disposition.

, Spent Fuel Storage Racks U' nit 1

An inspection of the spent' fuel storage rack assemblies was performed in order to assess the adequacy of the fabrication of the items. The Unit 1 4x4 and 7x7 assemblies have been installed in the containment ,

refueling pool and fuel building spent fuel pool. The work packages '

for these assemblies were reviewed in detail. The installation for the Unit 1 7x7 assemblies is complete except for the mock fuel insertion and withdrawal test (drag force). The package examined (1FH02-E8001) was complete with the exception of the closure of an NC The Unit 1 4x4 assembly.-installation is also complete including the insertion test. The document package reviewed for.this installation was IFH02-E8002. The mill test reports for all of the fuel cells were reviewed and found to be acceptabl Unit 2 The Unit II 4x4 and 7x7' assemblies were inspected in the storage yard, area G. The units are stored on dunnage and are secure.
