Licensee Action on Previously Identified Inspection Findings (Closed) Violation (498/499-8601-01): Failure to follow procedures for signing off QC hold points for postweld heat treatmen The NRC inspector reviewed the corrective steps which included:

3. Welding of Structural Steel Review of Procedures The NRC inspector reviewed the following procedures to assure that adequate controls have been established relative to welding of structural steel: o Standard Site Procedure SSP-16, General Structural Welding Requirements o ESI Procedure WP-5, Welding Procedure Specification o ESI Procedure WP-9, Welding Procedure Specification o ESI Procedure WP-10, Welding Procedure Specification o ESI Procedure WP-24, Welding Procedure Specification o ESI Procedure WP-43, Welding Procedure Specification o ESI Procedure WP-44, Welding Procedure Specification o ESI Procedure WP-50, Welding Procedure Specification o ESI Procedure WP-68, Welding Procedure Specification o ESI Procedure WP-111, Welding Procedure Specification o ESI Procedure WP-119, Welding Procedure Specification o ESI Procedure WP-1169, Welding Procedure Specification The NRC inspector identified to ESI his concern about some discrepancies between Weld Procedure WP-111 and the ESI Welding Procedure Identification Sheet, Form 84 Part F, in the areas of material specifications and preheat requirements. ESI reviewed the material specifications used at STP for construction of structural steel and determined that certain material specifications could be deleted or restricted for thicknes ESI issued a change to WP-111 and Form 84 Part F to resolve the discrepancies in material specifications and preheat. The clarifications provided by the changes to WP-111 and Form 84 Part F adequately address the concern expressed by the NRC inspecto Observation of Welding Activities The NRC inspector observed the welding and quality control activities of ESI associated with fabricating thirty category A Joint welds and 13 category B Joint welds in Unit 1 and 50 category A Joint welds in Unit 2.

In the areas-inspected for the above welds, the welding was accomplished in accordance with the welding procedure; the entries on the weld record for releasing QC hold points were consistent with the status of the weld; the welding filler materials were controlled consistent with SSP-30; welders were qualified for the procedure being used for welding; and with the exception of a small portion of one fillet weld being undersize, all completed welds were consistent with the visual acceptance criteria of SSP-16 for structural weld The fillet weld that had been ground undersize in excess of 1/16 inch for approximately 10 percent of the length of the weld was documented for tracing and disposition by ESI QC on NCR CC0352 ESI QC performed a reinspection of 30 additional welds of similar configuration and found no discrepancies on the size of the welds from the size required by the drawing. Based on the above inspection and the reinspection by QC, the undersized weld is considered to be an isolated case of inadvertent grindin No violations or deviations were identifie . Exit Interview The NRC inspector met with licensee representatives (denoted in paragraph 1) and Mr. C. Johnson, the NRC Senior Resident Inspector, on May 15, 1986, and summarized the scope and findings of the inspection. }}
