Areas Inspected: A routine, announced NRC independent measurements inspection was conducted using the NRC Mobile Nondestructive (NDE) Laboratory.
Selected safety related piping weldments fabricated to ASME Code,Section III, Class 1, 2 and 3, were inspected. Two regional based inspection personnel assisted by two contracted NDE personnel were utilized during this inspection.
Results: One violation was observed concerning debris in piping.
Paragraph 5.0 discusses a licensee commitment to perform a qualification test for magnetic particle examination on coated surfaces.

2.0 Independent Measurements - NRC Nondestructive Examination and Quality Records Review of Safety-Related Systems:
During the period of September 15 through September 26, 1986, quality records received from South Texas Nuclear Power Station, Units 1 and 2 were reviewed in the regional office for compliance to the licensee's FSAR commitaent to applicable codes, standards, specifications, and regulatory requirements.
Subsequently, an onsite independent verification inspection was conducted September 29 through October 9, 1986, using the NRC Mobile Nondestructive (NDE) Laboratory.
This inspection was performed by NRC regional personnel in conjunction with contracted NDE personnel.
The purpose of this inspection was to verify the adequacy of the licensee's Quality Control program.
This was accomplished by duplicating, as near as possible, those examinations required by codes and regulations, and evaluating the results.
In addition, several confirmatory examinations were performed and compared to quality assurance records.
The NRC inspectors performed the examinations on randomly selected weldments representing samples of piping systems, components, pipe sizes, vendor shop and field weldments fabricated to ASME Code Section III.
The items selected were previously inspected and accepted by the licensee, as indicated by vendor shop and onsite QA/QC records.
2.1 Quality Documents Review The thirty (30) safety-related piping system document packages listed in Attachment 4 were reviewed for compliance with licensee procedures, applicable codes, standards and regulatory requirements.
The following type documents were reviewed.
Document Attributes Reviewed Material Certification Material chemical and (base) physical properties compared to standards and Code requirements.
NDE Records Examinations meet Code and standards, licensee procedure and other commitments; appropriate examinations performed.
Fabrication Records Travelers and records were reviewed and compared against other corresponding records and signoff sheets.

2.2 Nondestructive Examination
}
Nondestructive examination was performed using NRC procedures with i
addenda written specifically to comply with the licensee's FSAR commitment to the ASME B&PV Code.
The intent of these examinations I
was to duplicate, to the extent practicable, the techniques and methods of the original examinations.
The specific items examined are shown in Attachment 1.
The following nondestructive examinations were performed:
Radiographic Examination Thirty safety related pipe weldments were radiographed using an Iridium 192 source.
The technique and procedure used were in I
accordance with NRC procedure, NDE-5, Revision 0 and addenda ST-1/2-5-1, Revision 0.
The weldments examined were ASME Class 1, 2 and 3.
The resulting radiographs were evaluated per applicable code requirements and compared to the licensee's corresponding radiographs.
Included in this examination were twelve socket welds examined for confirmatory purpose (engagement).
Results No violations were identified.
Liquid Penetrant:
Twenty-four safety-related pipe weldments and adjacent base material (1/2 in, either side of weld) were examined using color contrast liquid penetrant method per NRC procedure NDE-9, Revision 0 and addenda ST-9-1, Revision 0.
Samples included ASME Class 1 and 2 stainless steel welds.
Results:
No violations were identified:

Magnetic Particle:
Magnetic particle inspection was performed on three Residual Heat Removal (RHR) pump supports.
Weldments and adjacent base material (1/2 in either side of weld) were examined per NRC procedure NDE-6, Revision 0.
This sample included ASME Class NF, non-integral supports.

Results:
No violations were identified.
Thickness Measurements:
Twenty-five safety-related pipe weldments and adjacent base material were examined for minimum wall thickness using a NOVA-D 100 digital thickness gage.
Measurements were performed per NRC procedure NDE-11, Revision 0.
Minimum wall thickness was determined by use of ASTM standard pipe size and nominal thickness chart.
Results:
No violations were identified.
Ferrite Measurements:
Fifteen safety-related pipe weldments were examined for delta ferrite content using a type II ferrite indicator (Severn Gauge).
This test was intended to verify that the welding filler metal composition had achieved adequate (as-deposited) delta ferrite content to eliminate possible hot cracking.
Results:
No violations were identified.
Hardness Test:

l Eleven safety related pipe weldments and adjacent base material were tested using the Equo-tip hardness tester per NRC procedure NDE-12, Revision 0.
Measurements taken frcm tM :mpact device were processed into "L" values as a digital readout, the numbers (L" values) were then converted to Brinell hardr;e o le and approximate tensile strength by means of a conversitn c).
Results:
No violations were identified.

Instrument calibration was performed l
per NRC procedure NDE-2, Revision 0 (Instrument linearity verification).
Distance Amplitude Correction (DAC) was constructed using the licensee's

calibration blocks, SS-9-STP and SS-21-STP.
Instrument setting and search unit (transducer) were matched as near as possible to those
indicated by the ultrasonic data reports in an attempt to duplicate the original examinations.
Results:
No violations were identified.

Anchor bolt examination:
Forty-eight Hilti concrete anchor bolts as identified in Attachment 3 were ultrasonically inspected for length, and visually examined for location, type, size and length of bolt extended above concrete.
This examination was performed per NRC procedure NDE-9, Revision 0.
Included in this examination were sixteen cable tray supports and thirty-two (32) pipe hanger supports.

No violations were identified.
4.0 Radiographic Review
A sampling of the licensee's radiographs was reviewed. The inspector
reviewed radiographs for 29 shopwelds and 40 field welds to verify
'
compliance with ASME III Code requirements, accuracy of radiographic film interpretation and the adequacy of the licensee's radiographic programs.
l-Additionally, 30 welds were re radiographed by the NRC during this inspection and the resulting films were compared to site file films to independently verify that correct radiographic file films were on hand (see Attachment No. 2, items with asterisk).
Results:
Review of NRC radiographic film for weld Ril 2312-KB2-C disclosed that, within the pipe near the weld, debris such as wire and metal shavings was found.
Similar conditions were found for weld CV 2082-PB2-B, where radiographs disclosed several one-inch diameter metal washers and metal shavings.
Site Piping System Cleanliness procedure SSP-22 requirements have apparently not been met.
The inspector identified this problem as an apparent violation of 10 CFR 50 Appendix B, Criterion V for Unit 2 (499/86-17-01).
5.0 Preservice Inspection (PSI)
The inspector reviewed a sample of PSI ultrasonic data reports from:

During the review of the above PSI ultrasonic data, welds SI1208-BB1 FW1 and RH1101-BB1 SW7 were selected for independent ultrasonic examination by the NRC.
Results:
I During t.his PSI data review, it was revealed that Southwest Research Institute (SwRI) (PSI contractor) informed the licensee, through its customer notification number 058 SwRI, that procedure SwRI-NDT-300-1/30 for magnetic particle examination did not permit examination of welds that have not been made completely free of paint.
The licensee authorized SwRI to magnetic particle examine the welds for preservice inspection without completely removing the thin primer coat.
Welds in question were visually examined by the inspector.
The welds had been wire brushed in order to remove the primer coating.
However, in some areas there was a thin (1/2 mill measured by the licensee) adherent non-insulating, firmly affixed coating.
The inspector determined that this coating would not interfere with the formation of indications and, therefore the inspector considered this surface suitable for magnetic particle examination.
However, during the course of this inspection, a meeting was conducted with the licensee regarding magnetic particle examination over coated welds.
The outcome of this meeting was a commitment from the licensee to perform a qualification test on a surface with conditions similar to that of the welds examined and to verify the adequacy of the examination technique, with a completion date of the first week of January 1987.
This is considered an open inspection followup item and will be tracked by Region IV personnel.
