Exit Interview The inspection scope and findings were summarized on April 3,1981 with those persons indicated in paragraph 1 above. The inspector discussed the rationale for upgrading unresolved item No. 50-416/81-06-02 to a violation before closing it out. The licensee was also informed that the violation would not require a written response. The only new item in this report is that violation (50-416/81-07-01 - Incomplete QA Records).

Licensee Action on Previous Inspection Findings a.
(Closed) Unresolved Item (50-416/81-06-02) - Incomplete QA Records.
l The inspection conducted on February E3-26,1981, as reported in IE

Report 50-416/81-06, uncovered an ultrasonic test' report which did not have the blocks for equipment, transducer, test block, method used, scanning method, or sensitivity level filled out. 'The omission of this
,
information is in violation of the ASME B&PV Code 74S74 Edition,
'
Section V, Article 5, which states that reports shall identify proce-dures and equipment in sufficient detail to permit repetition of the examination.
As reported in IE Report No. 81-06, the licensee agreed that the UT report was deficient and that the inspection should be repeated. The l
licensee ~ ssued Corrective Action Request (CAR) No. 300 requesting that i
the contractor repeat the inspection of FW-48 with proper documenta-tion, review other UT data reports for similar deficiencies and take corrective action to prevent recurrence.

i The corrective actions taken by the contractor involved the repeat of the inspection of P' ~8; review of approximately 650 additional UT reports; and docu. ted reinstruction of all Bechtel and Peabody testing personnel invcived in generating and reviewing NDE reports.
The licensee verified. the completion of the corrective action by
independently r~1 ewing 15 randomly selected UT reports; Reviewing
Report No. P"...-704 which is the report of the reinspection of PJ-48; and interviewing two Peabody testing and four Bechtel WQCE personnel concerning the NDE reporting requirements.

After a review of the documentation used to close CAR 300 and discus-l sion with licensee representatives, the inspector informed the licensee

that this unresolved item would be reclassified as a violation, in that it was established that a requirement had not been met. The inspector

further stated that a response to the violation would not be required in that CAR 300:
'

(1) admits the violation;
(2) establishes it to be an i solated case of omission by a data
recorder; (3)
includes results of the repeated inspection with proper documen-tation; (4) documents the reinstruction of all personnel involved with gener-i ation and review of NDE reports; and (5) establishes that the licensee verified that full compliance had j
been achieved on March 31, 1981.

(Closed) Unresolved Item (50-416; 417/78-23-01) - Welding Procedure I
Review.
Inspection Report No. 78-23 noted that the licensee had
- nitiated a review of all Bechtel welding procedures to ensure that all essential variables were properly addressed in each procedure. This review was tracked by D&CR No. 1097, Supplement'1.
This D&CR was closed in January, 1979 when CAR 216 and CAR 217 w4re initiated by the licensee. The corrective actions required by CAR 216. and 217 have been completed and the licensee has cicsed the CARS. Based on..a review of IE Report 78-23; D&CR 1097, Supplement 1; and CARS 216 an'd 217, it appears that the subject of this unresolved item' was being actively pursued by the licensee's QA program.

(Closed) Unresolved Item (50-416/79-34-03, 50-417/79-30-01) - Speci-fication of Welding Technique and Heat Input Controls in Welding Procedures.
This is another area of the overall problem which the licensee was pursuing with CARS 216 and 217, as discussed above with unresolved item 50-416, 417/78-23-01. This item is closed.

The inspector reviewed the closecut of CAR 216 and CAR 217 which were the basis for closing D&CN 1997, Supplement 1..There are no further questions at this time.

Indicator Valves.
(MP&L PRD No. 80/42)~. The licensee has determined that this item is not reportable. The inspector reviewed the MP&L file on this item and lias no further questions at this time.

(Closed) Licensee Identified Item (50-416/80-20-15; 50-417/80-13-15) -
Feedwater Pipe Bend Wall Thickness (MP&L No. PRD 40). The inspector reviewed the licensee;s final report on this item.
There are no further questions at this t.me.

(Closed) Licensee Identified Item (50-416/81-07-02).
Recirculation Pump Hanger Components (MP&L No. PRD 80/65). This item was determinad to be not reportable. The inspector reviewed the manufacturer's repert on the acceptibility of the loose thread conditions.
The reporc included stress analysis considerations for normal and upset condi-tions. There are no further questions at this time.
7.
Extended Construction Delay (Unit 2)
The inspector discussed the licensee's program for protection of safety-related equipment and structures with members of the licensee's construction management and QA organizations.
The licensee is ac.t.ively researching preservation requirements for major items such as the reactor vessel as well as the many piping and structural components stored in the lay down areas.

The licensee also provided assurar.ce that when construction.resum'es on Unit 2, a full inspection and evaluation wil.1 be madenof all previously installed items and all items which have been in storage.

There were no vic1ations or deviations in this area of the inspection.

Containment (Steel Ltructures and Supports) - Unit 1 The inspector reviewed the installation of recirculation system pump supports. The following supports were selected for this inspection:

B302A B301B B302B These hangers were rigid supports between and imbed in the reactor pedestal and the pump casing. The inspector reviewed work plan and inspection record No. Q1M26-G-11136 V0A for the installation and inspection of "Q" structural steel including the supports mentioned above.
The fou-hangers were inspected for conformance with welding requirements and configuration requirements of the design drawings.
There were no violations or deviations in this area of the inspection.
9.
Reactor Coolant Pressure Boundary Piping (Unit 1)
'lhe RCPB piping has been installed and the system hydrostatically tested.
The inspector selected four steam drain lines inside the primary system hydro boundary to verify that the piping configuration agreed with the as-built drawings. The lines chosen were as follows:
