SUMMARY Inspection on January 2-30, 1981 Areas Inspected This routine, unatinounced inspection involved 80.5 inspector-hours on site in the areas of preoperational test witnessing, review of startup manual revision, RHR
"B" pump seizure, and plant tours.
Results Of the four areas inspected, no violations or deviations were identified.

Exit Interview The inspection scope and findings were summarized on January 30, 1981 with those persons indicated in Paragraph 1 above. The licensee had no comments regarding the inspection findings.

Preoperational Test Witnessing The inspector witnessed the conduct ef portions of preoperational tes.
procedure 1R21PT01, ESF 4.16 KV Preoperational Test. The test was witnesseJ for conformance with Startup Manual Chapters 5000 and 7000.
No violations or deviations were identified.

Review of Startup Manual Revision The inspector reviewed Revision 4 to the Mississippi Power and Light Company Startup Manual.
The revision was reviewed for conformance with ANSI'
18.7-1976 and FSAR Chapter 14. The following. discrepancies were noted.

a.
Open Item 416/81-03-01 The revision included a procedure for processing Temporary Test Oper-ating Instructions (TTOI). This instruction appears to duplicate the system run-in instruction included in SUM 5000. The instruction does not define the responsibility for determining if a TTOI is needed and no guidelines are provided for making this determination as required by ANSI 18.7-1976 Paragraph 5.1.15.

Open Item 416/81-03-02 Paragraph 4.1.4.8B of SUM 5000 appears to imply that a Temporary Test Operating Instruction would be required for recovery action in the event of an abnormal occurrence.
The SUM currently requires these types of events to be reviewed by the Startup Supervisor and evaluated by the Plant Safety Review Committee. This instruction could pos'.ibly circumvent these reviews and allow testing to continue without p cer management approval as required by ANSI-18.7 - 1976 Paragraph 4.3.* anc 5.2.11.
c.
Open Item 416/81-03-03 The instruction for Temporary Test Operating Instructions would allow an approval level that is not equivalent to that required for a pre-operational test procedure or a system operating instructica.
The instruction does not limit the changes or modifications that could be made with a TTOI.
Thus, a major change or revision could be imple-mented without management approval as required by ANSI 18.7 - 1976 Paragraph 3.1, 5.2.5 and 5.2.15.
d.
Other minor clerical and editorial errors were discussed with senior licensee management.
These errors were corrected by a subsequent advanced change notice.
There are no further questions remaining at this time concerning these items.
The implementation of the Temporary Test Operating Instruction has been withdrawn by 'the Project Startup Manager.
It is the inspector's under-standing from discussions with senior licensee management that the above open items will be reviewed and appropriate corrective action taken prior to implementation of the Temporary Test Operating Instruction. The inspector will review these corrective actions-during a subsequent inspection.
7.
RHR B Pump Seizure The inspector reviewed the documentation and interviewed Quality Assurance and startup personnel concerning the seizure of and damage to RHR pump "B".
The ' seizure occurred during the period immediately prior to the Reactar Pressure Vessel hydrostatic test.
The event occurred subsequent t.o an overpressurization of the RHR system due to equipment failure.

has not been determined. The licensee has reported the event to NRC Region II. This was documented by PRD 81-02.
Unresolved Item 416/81-03-04

The RHR system, reactor pressure vessel and associated fluid systems had previously undergone cleaning and flushing operations prior to this event.
All three RHR pumps had cleanliness strainers installed in the suction lines of the pump. The PRD indicated that subsequent to the overpressurization event, pumps "A", "B" and "C" could not be turned by hand, indicating that similar problems may be developing in pumps "A" and "C".
It does not, however, document or discuss the actions necessary to assure that RHR pumps
"A" and "C" have been inspected to ensure they do not contain foreign'
,
material which will lead to subsequent failure during operation. This item will be held open pending investigation, review and correction by the licensee of potential internal cleanli.,ess and damage problems which could
affect operability of RHR pumps "A" and "C".

The inspector reviewed a revision to the Startup Manual which added the requirement for Plant Quality to review completed safety related pre-operational test procedures. This item is closed.
