APPENDIX _A NOTICE OF VIOLATION Mississippi Power and Light Company Docket Nos. 50-416 Grand Gulf License Nos. CPPR-118 As a result of the inspection conducted on March 31 - April 3,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980),

the following violation was identified.

10 CFR 50, Appendix B, Criterion XVII requires that sufficient records shall be maintained to furnish evidence of activities affecting quality. The ASME B&PV Code, 1974 Edition with Summer 1974 Addenda,Section V, Article 5, Paragraph T-539 requires that ultrasonic inspection test reports shall identify procedures and equipment sufficiently to permit repetition of the examination.

Contrary to the above, the requirement to record procedure and equipment data was not met in the case of the report of the ultrasonic inspection of FW-48 on drawing FSK-P-305-M1312D as reported in Peabody x-ray engineering report no. PBT-UT 185, in that the equipment, transducer, test block, method, scanning method and sensitivity level information blocks were not filled in and the report had been accepted by the licensee's agent and filed in the QC records vault.

This is a Severity Level VI Violation (Supplement II.F).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this cffice within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violation; (2) the reasons for the violation if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date w' Atomic Energy Act of hen full compliance will be achieved. Under the authority of.Section 182 of the 1954, as amended, this response shall be submitted under oath' or affirmation.
