Dear Robert Schuetz:

On September 30, 2023, the U.S. Nuclear Regulatory Commission (NRC) completed an inspection at Columbia Generating Station. On October 12, 2023, the NRC inspectors discussed the results of this inspection with Dave Brown, Operations Vice President, and other members of your staff. The results of this inspection are documented in the enclosed report.

No findings or violations of more than minor significance were identified during this inspection.

SUMMARY

The U.S. Nuclear Regulatory Commission (NRC) continued monitoring the licensees performance by conducting an integrated inspection at Columbia Generating Station, in accordance with the Reactor Oversight Process. The Reactor Oversight Process is the NRCs program for overseeing the safe operation of commercial nuclear power reactors. No findings or violations of more than minor significance were identified.

PLANT STATUS

Columbia Generating Station (CGS) began the inspection period at rated thermal power (RTP).

On July 2, 2023, the unit reduced power to 85 percent for emergent adjustable speed drive (ASD) maintenance. On July 4, 2023, ASD maintenance was completed, and the plant returned to 100 percent RTP. On July 10, 2023, the unit was again reduced to 85 percent for planned ASD maintenance. Maintenance was completed on July 11, 2023, and the unit returned to 100 percent RTP. On August 3, 2023, the unit was reduced to 65 percent for planned ASD maintenance and returned to 100 percent RTP that same day. On September 20, 2023, the unit emergently decreased power to 92 percent due to issues with the condensate demineralizer system and following maintenance, the unit was returned to 100 percent RTP on September 21, 2023. On September 23, 2023, the unit was lowered to 80 percent for planned surveillance testing and returned to 100 percent RTP that same day. The unit remained at RTP for the rest of the inspection period.

INSPECTION SCOPES

Inspections were conducted using the appropriate portions of the inspection procedures (IPs) in effect at the beginning of the inspection unless otherwise noted. Currently approved IPs with their attached revision histories are located on the public website at http://www.nrc.gov/reading-rm/doc-collections/insp-manual/inspection-procedure/index.html. Samples were declared complete when the IP requirements most appropriate to the inspection activity were met consistent with Inspection Manual Chapter (IMC) 2515, Light-Water Reactor Inspection Program - Operations Phase. The inspectors performed activities described in IMC 2515, Appendix D, Plant Status, observed risk significant activities, and completed onsite portions of IPs. The inspectors reviewed selected procedures and records, observed activities, and interviewed personnel to assess licensee performance and compliance with Commission rules and regulations, license conditions, site procedures, and standards.

REACTOR SAFETY

71111.01 - Adverse Weather Protection

Seasonal Extreme Weather Sample (IP Section 03.01) (1 Sample)

(1) The inspectors evaluated readiness of the adjustable speed drive A and B standby glycol heat exchanger fans and normal transformer TR-N1 and TR-N2 cooling sprays for extreme weather conditions prior to the onset of seasonal high temperatures on August 17, 2023.

71111.04 - Equipment Alignment

Partial Walkdown Sample (IP Section 03.01) (3 Samples)

The inspectors evaluated system configurations during partial walkdowns of the following systems/trains:

(1) residual heat removal system C when low pressure core spray was out of service for maintenance on July 10, 2023
(2) high-pressure core spray system when reactor core isolation cooling system was out of service for maintenance on September 12, 2023
(3) standby gas treatment system A while standby gas treatment system B was out of service for maintenance on September 26, 2023

Complete Walkdown Sample (IP Section 03.02) (1 Sample)

(1) The inspectors evaluated system configurations during a complete walkdown of diesel generator 3 on September 20, 2023.

71111.05 - Fire Protection

Fire Area Walkdown and Inspection Sample (IP Section 03.01) (4 Samples)

The inspectors evaluated the implementation of the fire protection program by conducting a walkdown and performing a review to verify program compliance, equipment functionality, material condition, and operational readiness of the following fire areas:

(1) fire area SW-1, standby service water pump house A, on August 10, 2023
(2) fire area RC-10/U, control room, on August 10, 2023
(3) fire area 5010-3, secondary essential fire suppression water supply building 33, on August 22, 2023
(4) fire area R-3, high-pressure core spray pump room, on September 12, 2023

Fire Brigade Drill Performance Sample (IP Section 03.02) (2 Samples)

(1) The inspectors evaluated the onsite fire brigade training and performance during an announced fire drill on July 13, 2023.
(2) The inspectors evaluated the onsite fire brigade training and performance during an unannounced fire drill on September 26, 2023.

(2) condensate demineralizer inlet valve C repair and replacement, following failure on September 19, 2023

(2) CR 449521, one service water B spray ring header nozzle clogged in the service water A pond, completed on August 21, 2023
(3) CR 448300, reactor water sample line inner isolation valve RRC-V-19 failed to isolate flow during quarterly inservice testing, on August 31, 2023
(4) CR 450082, high-pressure core spray pump 2 discharge valve, SW-V-29, actuator mounting bolts found with less thread engagement than specified in plant procedure 10.2.10, on September 11, 2023
