On 12/25/85 at 1713, with the unit in operational condition 2 (startup) .and during the performance of a surveillance test procedure, a Reactor Water Cleanup (RWCU) system isolation occurred. The cause was attributed to an error in the surveillance test procedure. The performance of the procedure was terminated and the RWCU system was returned to service with no further problems occurring. The procedure was revised and successfully completed on 12/27/85. There wae no impact on the safe operation of the plant or the health and safety of the public since no identifiable leakage condition existed within the RWCU system.

• r 5:S i85 - Q6liff0,0 0,2c,o.3 On 12/25/85 at'1713 with the unit in operational condition 2 (startup), an inadvertent Reactor Water Cleanup (RWCU) (IEEE:CE) isolation occurred during the performance of Surveillance Test Procedure STP-207-4218 "RWCU System Isolation Differential Flow Timer Quarterly Channel Calibration." The isolation occurred during the placement of a test switch and was a result of an error contained in the procedure. The switch was removed and the performance of the surveillance test procedure was terminated. The

RWCU system was returned to service with no further problems occurring.

Investigation revealed that the procedure failed to require switch E31-SlA (RWCU Bypass Switch) to be placed in the bypass position while connecting the required test equipment. As directed by the procedure, the Instrument & Controls technicians placed a test switch across terminals 9 and 10 on TB12 for temperature switch E31-N609A. These terminals are located immediately adjacent to each other. During the installation of the switch the connections were shorted causing the inadvertent isolation.

In an effort to prevent recurrence, Surveillance Test Procedure STP-207-4218 was revised per Temporary Change Notice 85-4487 to require switch E31-SlA to be placed in the bypass position prior to installing the required test equipment. As a result of an additional RWCU isolat' ion occurring during the performance of STP-207-4218 on 12/26/85 (reference LER 85-062) , other procedural

There was no impact on the safe operation of the plant or the health and safety of the public since no identifiable leakage condition existed within the RWCU system.
