SUBJECT: NRC INSPECTION REPORT NO. 50-458/99-04
Dear Mr. Edington:
This refers to the inspection conducted on April 12-16 and April 28-29,1999, at the River Bend
Station facility. On June 2,1999, a telephonic exit meeting was held to discuss the results of
the inspection. The enclosed report presents the results of this inspection.
Based on the results of this inspection, the NRC has determined that five Severity Level IV
violations of NRC requirements occurred. These included: (1) a violation for the failure to
perform a prejob briefing in accordance with procedural requirements; (2) a violation with five
examples involving personnel failing to adhere to radiation work permit requirements; (3) a
violation with two examples involving the failure to adequately inform workers; (4) a violation for
the failure to lock or continuously guard a locked high radiation area; and (5) a violation for the
failure to maintain the appropriate personnel qualified to wear respirators. These violations are
being treated as noncited violations (NCVs) consistent with Appendix C of the Enforcement
Policy. These NCVs are described in the subject inspection report. If you contest the violation
or severity level of these NCVs, you thould provide a response within 30 days of the date of this
inspection report, with the basis for your denial, to the Nuclear Regulatory Commission, ATTN:
Document Control Desk, Washington DC 20555-0001, with copies to the Regional
Administrator, U.S. Nuclear Regulatory Commission, Region IV,011 Ryan Plaza Drive,
Suite 400, Arlington, Texas 76011, the Director, Office of Enforcement, United States Nuclear
Regulatory Commission, Washington, DC 20555-0001; and the NRC Resident inspector at the
River Bend Station facility.
The findings documented in this report, NRC Inspection Report 50-458/98-10, and your recent
assessments in this area are indicative of potentially programmatic problems associated with
your ability to effectively inform, control, and protect radiation workers at your facility.
Consequently, we would like to discuss the actions you have taken or planned to address these
problems at a management meeting in the Region IV offices. This meeting has been
scheduled for July 26,1999, at 12:30 p.m. (CDT) as discussed in the June 24,1999, meeting
notice. During this mesling, we request that you specifically address: (1) corrective actions for
the NCVs noted in this report at well as NRC Inspection Report 50-458/98-10, (2) actions taken

or planned to improve the capability to identify and document radiation protection program
implementation problems, (3) the results of any assessments completed in this area
subsequent to this inspection, and (4) any other actions taken or planned to improve the
implementation effectiveness of the radiation work permit program and provide adequate
instructions to radiation workers.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter, its
enclosure (s), and your response, if requested, will be placed in the NRC Public Document
Room (PDR).
