Jones during the period November 1-30, 1989, of activities authorized by NRC Operating License NPF-47 for River Bend Station - Unit 1. GSU's response to the violation is provided in the enclosure.

.. . RESPONSE TO NOTICE OF VIOLATION 50-458/8940-01 LEVEL IV REFERENCE Notice of Violation - Letter from S. J. Collins to J. C. Deddens, dated January 10, 1990.

FAILURE TO PROVIDE ADEQUATE CONTROL FOR MATERIAL ACCESS TO THE CONTAINMENT The Code of Federal Regulations, in 10CFR50.59, allows a licensee to make changes to a facility as described in the safety analysis report provided -

the changes do not increase the probability of a malfunction of equipment important to safety.

Contrary to the above, on November 30, 1989, with the reactor at 97 percent thermal power, the licensee made a change to the facility by placing materials in the containment which increased the probability of a malfunction of emergency core cooling systems during an accident in that the material could obstruct the flow path of those emergency systems that use the suppression pool as a source of cooling water for the reactor.

-These materials included several hundred square feet of plastic sheeting, computer terminals and tables located on a walkway directly over the suppression pool.

-REASON FOR THE VIOLATION On November 30, 1989, the Director - Radiological Programs initiated Condition Report (CR) 89-1242. This CR identifies a condition where materials were pre-staged on the 95' elevation of the containment in preparation for an outage without approval of the shift supervisor. The root cause of this event is considered to be a lack of coordination between the shift supervisor and the radiation protection personnel responsible for placing this material inside the containment.

CORRECTIVE STEPS WHICH HAVE BEEN TAKEN AND THE RESULTS ACHIEVED Upon discovery, all pre-staged material was removed from the containment.

On December l'1, 1989, the Director-Radiological Programs issued a memorandum to all radiation protection personnel reminding them to notify the shift supervisor of the quantity and location of material being moved into the containment. Additionally, all Radiation Protection personnel were cautioned on the placement of material on the 95' elevation of the containment.

-A sign will be posted inside the containment at the entrances to the 95' elevation stating: ,

WITH THE PLANT- IN MODES 1 OR 2, MATERIALS BEING USED ON THE 95' ELEVATION SHALL NOT BE LEFT UNATTENDED UNLESS PROPERLY ANALYZED.

.An additional sign will be posted at the airlock ^ entrances reminding personnel that any irretrievable item (s) dropped into the suppression pool i shall be. identified on a condition report in accordance with Administrative -

Procedure (ADM)-019.
