River Bend Station - Unit 1 Refer to : Region IV Docket No. 50-458/ Report 86-04 This letter supplements Gulf States Utilities Company's (GSU)

April 11, 1986 response to the Notice of Violation contained in Inspection Report No. 50-458/8604-01, " Failure to Administratively Control Temporary Circuit Alterations." To correct the unauthorized use stated: "As of January 7, 1986, Operations Qualityof lifted leads Control QC) and jumpers,(GS has instructed QC Planners and Lead Inspectors to include a hold point on Electrical Maintenance Work Requests for Category I Electrical Equipment Panels. This will allow for inspection upon completion of work for ' lifted leads, jumpers, cleanliness, proper termination and separation'." ,

A review of maintenance history and QC Inspection Reports since revising Operations Quality Procedures and Instructions (QCI)-3.7, " Quality Control Inspection Planning", which includes a section describing the use of the QC Planning Review Checklist to establish QC Hold Points where required or applicable, and General Maintenance Procedure (GMP)-0042, " Circuit Testing and Lifted Leads and Jumpers", which requires the use of serialized tagging to provide better control and accountability for lifted leads and jumpers, has shown a steady decline in the number of unsatisfactory conditions noted by QC, GSU is confident that this trend will continue. GSU is modifying instructions for QC inspections of electrical equipment panels as stated below.

" Quality Control, on a quarterly basis, will conduct a random sampling of all accessible control room panels during normal I

plant operation. Additionally, a random sampling of Q-Class l 1 electrical panels located in the power block will be l

conducted prior to plant start-up following any planned outage."

included in the original . response to.the Notice of Violation along with this modified action will prevent recurrence of future i l violations.  ;
