On August 5,1999, a management meeting was held witti representatives of the River Bend Nuclear Station in the Region IV Office, Arlington, Texas, to discuss the radiological control problems identified in NRC Inspection Reports 50-458/98-10 and 50-458/99-04. The meeting was held at the request of the NRC.

The licensee presented their Site Radiological improvement Plan which addressed the concerns raised in the above NRC inspector reports.

The attendance list and the licensee's presentation are enclosed with this summary (Enclosures 1 and 2). ,

in accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its enclosure will be placed in the NRC Public Document Room (PDR). i l

O SYNOPSIS Entergy Operations incorporated is committed to being one of the nation's

! premier nuclear operating companies. This vision is part of the company's core l

business strategy for the future. In order to be a premier nuclear operating l company, EOl must exhibit exemplary performance in the processes, procedures and programs associated with nuclear power operations. Appropriate i radiological work practices are fundamental to such performance.

The Site Radiological improvement Plan was designed to identify the components of good radiological work performance. Not all of the identified components have Action items associated with them. The Action items were assigned to those components which were identified as being in need of improvement. As this model is used to improve radiological performance, additional actions may be identified for the other components. These actions would be added to the plan at that time. Changes in the plan's scope will require approval by the Superintendent-Radiation Control, the Manager Technical Support and the General Manager Plant Operations.

The components which influence good radiological work performance is l categorized in the following areas.

The actions outlined in this improvement plan are designed to address these issues and 10 focus the site's attention and resources on the effective implemetation of its Radiation Protection Program, specifically in the areas identified as_having shortcomings. This is a "living" document. As Action items are completed or the need arises for the creation of additional Actions items, this document will be adjusted accordingly to meet our objective of improving the -

Radiation Protection Program at River Bend Station.

River Bend Station is cognizant of its responsibilities of informing and protecting )

its work force and the public, in regards to radiation exposure. Therefore, the station is committed to demonstrating excellent performance in the implementation of its Radiation Protection Program. This improvement Plan is designed to address the issues identified in specific focus areas and enhance the performance of station personnel in the implementation of radiological work practices.

' This plan contains River Bend Station's initiatives for improving the implementation of selected areas of its Radiation Protection Plan. This is a site-wide issue which requires a site-wide response and our success requires a common commitment to this mission. Accordingly, the plan requires actions to be owned and completed by multiple departments and entities. The plan provides a single point of reference for the status of these actions.

The plan is divided into Focus Areas. Each Focus Area then contains a number of Action items. Each Action item is assigned an owner or owners who will accomplish the task.

Quality Assurance observations of job turnovers, work group turnovers, shift turnovers, and pre-job briefings were increased for the time period of 4/15199 to 4/20/99. No significant issues were identified during the observations. This review was exited as Surveillance 904001 with senior management as part of the surveillance review / approval process. The results of this surveillance Indicated that RWP pre-job briefings observed were conducted in an appropriate manner.

An additional Action item in this Improvement Plan addresses revising the Quality Assurance, monitoring plan to include RWP pre-job briefings and Radworker practicos with added emphasis on supervisor oversight of radiological work (Action item #3.3)

Work Permits, Revision 16, were reviewed for completion of the pre-job briefing O checklist. Those that were incomplete were completed to reflect the checklist as it would have been, were it completed prior to the initial brief. This was accomplished for all of the RWPs except two. In those cases, the identity of the individuals who gave the initial brief is indeterminable and no corrections were possible.

The multi-discipline team was established to review the station's implementation of its Radiological Protection Plan. The team was composed O of members from Maintenance, Training, Safety & Regulatory Affairs, Operations, and Radiation Protection as well as Radiation Protection personnel from two othar EOl sites. The review was conducted from 5/3/99 through 5/6/99 and illuminated weaknesses in the areas of Management Oversight, Radworker Practices and the Radiation Protection Department's performance (notably the RWP process). The team's recommendations were reviewed by the General Manager- Plant Operations and were used as the basis for developing the station's improvement Plan.

Scope:

The multi-discipline team was formed, comprised of representatives from Radiation Protection, Mechanical Maintenance, Maintenance Planning, and an experienced subject matter expert on RWPs from Waterford Ill. The following is a synopsis of the changes proposed by the RWP review team.

= Clarify the requirements for pre-job briefings and define the process for administering this program. This process requires both the RP supervisor and Job supervisor to present the briefing to the workers. A revised ]

checklist was created that will be consulted during the planning phase of the work.

= Establish a formal mechanism for deviations from the written RWP requirements. This is a new form that is part of the RWP and requires RP supervisor approval for deviations. During the revision process any outstanding deviation is incorporated into the RWP.

= Revise the flow chart for the RP planning process to coincide with the procedure steps.

= Clarify the ALARA committee review requirements and approvals.

= Further clarify the use of the RWP history sheet.

The group also reviewed the station's procedure governing access control to the Controlled Access Area. Changes to this procedure have also been drafted to correspond with the proposed changes to the RWP procedure. These changes included differentiating between formal ALARA pre-job briefings and routine CAA access radiological briefings. This procedure will be issued concurrently with the O revised RWP procedure.

The equipment and processes used in the plant will be duplicated in this training. Training will begin with a classroom phase to introduce the Radworker Expectations and Guidelines.

The training will include the use of a series of mock-ups to place the student inta various postulated radiological plant conditions. Each mock-up will require the student demonstrate appropriate radiological work practices for the varying plant conditions.

The performance of the students and supervisor personnel will be evaluated against the established Radworker Er.pectations and Guidelines.

Scope:

Integrated Training places Radiation Protection and craft personnel in simulated radiological work environments for specific maintenance tasks.

The training includes the appropriate radiological briefings, hold points, and mock-ups of equipment to simulate plant conditions. It allows Radiation Protection personnel to become familiar with maintenance tasks, and for j craft personnel to practice radiological controls and industrial safety practices in a simulated work environment. These training sessions are critiqued and lessons learned are fed back into the maintenance and RP planning processes as well as the appropriate training programs.

Scope:

Currently the procedure allows this practice with the approval of the Superintendent-Radiation Control. The procedure revision is to provide more guidance as to when this practice is applicable and the constraints for its use.

The station failed to completely meet the respiratory qualification requirements of PL-026, Respiratory Protection Policy. This procedure states that 100 percent cf medically capable radiation protection technicians will be qualified to wear respiratory equipment. In this instance, three Radiation Protection technicians were discovered to have expired quantitative fit tests. One technician was not able to be qualified due to medical reasons, and therefore should be considered excluded from the requirements R-PL-026. One of the technicians was at the Waterford 3 outage for the month the qualifications expired and upon his return no follow-up tracking and communication was performed. The other technician was on site, but due to scheduling conflicts did not obtain a fit test. The RP supervisor task schedule requires that qualification status be determined on a monthly basis. Proper notification was made but follow up tracking was not adequately performed.

A similar situation involving Operations personnel was also noted. The station provides qualification data, including respiratory qualifications, via its local computer network. In these cases, the qualification data was available but supervision did not ensure the qualifications were maintained.

Operations has taken actions to address this issue.
