During_the Chemistry Department's routine inspection of plant radiation monitors, it was discovered at 0820 on 1/8/87, with the unit in full power operation, that the Main Flant Exhaust radiation monitor (RMS-RE125) particulate and iodine sampler was isolated. At 1220 on 1/8/87 it was discovered that the Fuel Building Exhaust radiation monitor (RMS-RESA) particulate and iodine sampler was also isolated.

Per Technical Specification Table 3.3.7.11-1 items 2.b and 2.c, the requirement to continuously monitor the exhaust ventilation systems for particulate and iodine activity was not met within the 8 hours9.259259e-5 days <br />0.00222 hours <br />1.322751e-5 weeks <br />3.044e-6 months <br /> allowed by the Action Statement. Upon discovery, the sample flows l were immediately restored to the sampler. Investigation determined that the root cause was due to the operator failing to notify the Chemistry Department prior to changing the sample alignments.

Safe operation'of the plant and the health and safety of the public were not affected as a result of this event. During the time the samples were not being collected there were no inadvertent releases to the environment. Additionally, there is no spent fuel stored in the Fuel Building at this time.

A similar event of a missed sample due to improper sample lineup was reported in LER 86-046.

INVESTIGATION r i

i During the investigation of the event it was determined that on 1/7/87 j at 2055 the Main Plant Exhaust radiation monitor (RMS-RE125) was remotely realigned to clear a " Loss of Isokinetic Control" alarm to an alternate sample path such that the particulate and iodine sampler was inadvertently isolated. Later that day at 2300 the Fuel Building ,

Exhaust radiation monitor (RMS-RESA) was similarly remotely realigned to clear a " Loss of Process Flow" alarm. On each of these occasions the operator failed to notify the Chemistry Department of the sample alignment change.  ;

CORRECTIVE ACTION Immediate action was taken to issue an " operator aid" which was attached to the front of each Digital Radiation Monitoring System (DRMS) monitor with a sample selection function switchable from the 1 Main Control Room. The operator aid states specific instructions for I

notifying the Chemistry Department prior to switching sample i alignment.

1 Additionally, a memorandum was issued by the Operations Supervisor to all Operations Section personnel noting the requirement to notify the Chemistry Department prior to changing sample alignments.

l l SAFETY ASSESSMENT l

There was no impact on the safe operation of the plant or to the I health and safety of the public. During the time that the samples

! were not collected for particulate and iodines there were no I

inadvertent releases to the environment. Additionally, there is no spent fuel stored in the Fuel Building at this time.
