Dear Mr. Edington:

This refers to the inspection conducted May 10 to 28,1999, at the River Bend Station facility,
with followup and inoffice inspection until July 1,1999. The purpose of the inspection was to
review a previous NRC concern relating to testing and cleaning of the auxiliary building unit
coolers. The enclosed report presents the results of the inspection.
Based on the results of this inspection, the NRC has determined that three Severity Level IV
violations of NRC requirements occurred. These violations are being treated as noncited
violations, consistent with Append;x C of the Enforcement Policy. If you contest the violations

or severity level of these noncited violations, you should provide a response within 30 days of
the date of this inspection report, with the basis for your denial, to the Nuclear Regulatory
Commission, ATTN: Document Control Desk Washington, DC 20555-0001, with copies to the
Regional Administrator, Region IV, and the Director, Office of Enforcement, United States
Nuclear Regulatory Commission, Washington, DC 20555-0001; and the NRC resident inspector
at the River Bend Station.
The findings from this inspection revealed that problems involving the design capability of a
number of safety-related room coolers were not resolved in a timely manner. Specific findings
from this inspection included examples of failing to: (1) resolve room cooler test result
deficiencies; (2) perform adequate operability determinations; (3) initiate condition reports for
conditions adverse to quality; (4) maintain the licensing basis current; (5) make appropriately
conservative judgments regarding the implementation of the Generic Letter 89-13 program; and
(6) sufficiently justify or document engineering conclusions and evaluations.

Other recent NRC inspections have identified instances of failing to resolve long-standing,
safety-related equipment problems (NRC Inspection Report 50-458/98-13) and instances of not
identifying or not documenting long-standing radiological work permit program implementation
'
problems in condition reports (NRC Inspection Report 50-458/98-10). This latter issue will be

one of the subjects to be discussed by NRC and Entergy Operations, Inc., during a meeting,
which is scheduled to be conducted in the Region IV Office on August 5,1999.

The findings from this inspection, as well as other recently completed NRC inspections, are
indicative of potentially broad-based problems associated with the ability to effectively identify,
analyze, and correct both hardware and programmatic deficiencies. Accordingly, we encourage
you to review the overallimplementation effectiveness of your program and processes for
identifying, analyzing, and correcting problems.
In accordance with 10 CFR 2.790 of the NRC's " Rules of Practice," a copy of this letter and its
enclosure will be placed in the NRC Public Document Room (PDR).
