This letter transmits TMI-l Licensee Event Report (LER) No. 98-009-00 regarding a potential ,

loss of High Pressure Injection (HPI) during a postulated loss of coolant accident (LOCA) with j the most limiting single failure. The root cause of this event was a misapplication of design inputs when operating procedures were changed prior to startup for the current operating cycle which ,

began in October 1997. This condition was determined to be reportable in accordance with l 10CFR50.72(b)(2)(iii) and notification was made via the ENS telephone on August 20,1998 as a  !

l condition that alone could have prevented the fulfillment of the safety function. This LER is being f ,
