cvaluated and determined to be reportable on January 22,1999 as a condition outside the design bases in

- accordance with 10 CFR 50.72(b)(1)(ii)(B). This LER is a follow-up to the notification made via the ENS
- telephone to the NRC Operations Facility on January 22,1999.
- This LER is being submitted pursuant to 10 CFR 50.73(a)(2)(v), using the required NRC forms (attached).
